上颌窦霉菌病36例临床分析

Clinical analysis of 36 cases of fungal maxillary sinusitis

目的 :探讨上颌窦霉菌病的诊断及治疗方法。方法 :分析 36例上颌窦霉菌病患者的临床资料 ,对 36例患者行鼻内窥镜和柯 陆氏手术联合进路手术及单纯鼻内窥镜手术治疗 ,术后病理确诊。结果 :36例均获治愈 ,行双径路 (鼻内窥镜加柯 陆氏手术 )术式 2 7例 ,其中 2例术后冲出残留的霉菌团块 ,而行单径路 (鼻内窥镜手术 )术式9例 ,其中 6例术后冲出残留的霉菌团块。结论 :上颌窦霉菌病以非侵袭型最常见 ,鼻窦CT是早期发现病变的有效方法 ,治疗的关键是解决鼻窦的通气和引流 ,Fess是治疗非侵袭性上颌窦霉菌病的有效方法 。

Objective: To investigate the diagnosis and treatment of fungal maxillary sinusitis Methods: The clinical materials of 36 cases with fungal maxillary sinusitis were analysed, 36 patients were treated by using endoscopic and Caldwell Luc′s operation The pathological diagnosis was obtanined after operation Results: All cases were cured, 27 cases were undergone functional endoscopic sinus surgery (FESS) and classical Caidwell Luc′s operation, another 9 cases were undergone functional endoscopic sinus surgery The remnant of fungus of double pathway (FESS and Caldwell Luc's operation) was 2 cases while single pathway (FESS) was 6 cases Conclusion: The non invasive mycosis of maxillary sinus are common CT of paranasal sinus is an effective way in diagnosis The key is to solve the ventilation and drainage of paranasal sinus FESS is effective treatment to non invasive mycosis of maxillary sinus The operation of double pathway is superior to that of single pathway
