孕、产妇外周血和胎儿脐血骨钙素水平的动态变化

摘要 目的 为阐明孕妇和胎儿骨钙素 (osteocalcin ,Ost)代谢规律 ,对各孕周妇女外周血和胎儿脐带血Ost水平的动态变化进行研究。方法 采用荧光酶联免疫法测定 14 2例孕妇、2 1例非孕妇女、2 2例产妇及 34例不同孕龄胎儿的脐带血Ost水平。结果 孕妇血清Ost水平随妊娠发展逐渐下降 ,至孕中期最低 [(3.30± 1.4 6 )μg/L],随后又逐渐升高。分娩即刻产妇血清Ost水平 [(12 .81± 4 .2 8) μg/L]显著高于妊娠各阶段的水平 (P <0 .0 1) ,产后数日即显著下降至接近妊娠前的基线水平。脐带血Ost水平在孕中期最低 [(0 .88± 0 .39) μg/L],随后则升高 ,并且自孕 2 4周保持于接近母体Ost水平 (P <0 .0 1)。结论 由于这段妊娠期是胎儿Ost积累活跃的阶段 ,因此这种恒定的Ost水平对胎儿骨骼系统的正常发育 。 Objective To clarify maternal and fetal osteocalcin metabolism by investigating dynamic variation of osteocalcin levels in maternal peripheral blood and fetal cord blood. Methods By routine methods, we measured serum concentration of osteocalcin in 142 pregnant women, 21 nonpregnant women and 22 postparturient women, as well as cord blood levels of osteocalcin in 34 infants of various gestational ages. Results Maternal osteocalcin level declined toward mid pregnancy[(3.30±1.46) μg/L] and increased thereafter. Serum level of osteocalcin at delivery[(12.81±4.28) μg/L] was significantly higher than those throughout pregnancy( P <0.01), but it dramatically decreased to around the baseline level of nonpregnancy in several days postpartum. Cord blood level of osteocalcin was the lowest during mid pregnancy [(0.88±0.39) μg/L] and increased thereafter. It remained similar to maternal osteocalcin level from 24 weeks through 36 weeks of gestational age ( P <0.01). Conclusions The level of osteocalcin may be important to the development of fetal skeletal system, because calcium accumulation by the fetus is activating during this gestational period.
