摘要
目的 :提高椎体间融合的成功率 ,减少植骨块移位、塌陷造成的并发生症。方法 :设计了一套特殊器械 ,从髂骨上取出圆柱状骨块并用螺纹成形器制成螺纹柱状骨块。在融合区减压孔内用攻丝器制出内螺纹 ,用叉持器将螺纹柱状骨块旋拧入融合区 ,完成融合术。结果 :从 1995年至 2 0 0 2年 ,用此方法完成颈椎椎体融合术 33例 ,腰椎椎体间融合术 15例 ,全部融合成功。结论 :螺旋柱状骨块较其他形状的骨块有明显的优点 ,可提供稳定的椎体间融合 ,避免因骨块移位而产生的各种并发症 ,还由于螺旋柱状块增大了表面积 ,可增加抗压强度 。
Objective: To improve the rate of success of the fusion and reduce the complications due to displacement of the bone graft.Method: A set of special instruments was designed, a threading tool can make a threaded Cylindrical bone from the iliac crest. After creating the intervertebral orifice, an inner threaded is made on the walls of the orifice with a screw tap. The threaded bone is screwed into the orifine by means of a fork.Result: Between 1995 and 2002, we operated on 32 patients with cervical spine pathology and 15 cases with lumbar vertebrae diseases, all of the cases got successful fusion.Conclusion: The threaded cylindrical bone graft can offers considerable advantages over bones of other shapes. It has a great degree of spinal fusion and stabilization. Thus avoiding the rupture of the graft bone and neural contusions. It has a greater surface in contrast with the vertebras, a greater resistance to pressure and less risk of collapse on angulation can be got.
