幽门螺杆菌相关性十二指肠溃疡短程三联治疗疗效的临床研究

Clinical study of the effect of short term triple therapy on Hp-related duodenal ulcer

目的 :研究短程三联治疗幽门螺杆菌相关性十二指肠溃疡的疗效。方法 :胃镜病理检查证实十二指肠单发溃疡活动期 (A期 )伴 Hp感染的患者分为一周组及维持组 ,均予三联抗幽门螺杆菌治疗。一周组治疗一周后即停止一切药物治疗 ,维持组则继续抗酸治疗四周 ,之后停药。两组均于治疗结束后四周及一年后复查胃镜及病理检查 ,了解所有患者溃疡的愈合及幽门螺杆菌根除情况。结果 :经三联抗幽门螺杆菌治疗结束后一年复查胃镜及病理检查 ,一周组溃疡愈合率 83% ,幽门螺杆菌根除率 75 % ;维持组溃疡愈合率 94 % ,幽门螺杆菌根除率 80 %。两组愈合率及幽门螺杆菌根除率相比较 ,差异无显著性。经三联抗幽门螺杆菌治疗结束后一年 ,Hp根除组溃疡愈合率 96 % ,Hp未根除组溃疡愈合率 6 4 % ,差异有显著性。结论 :治疗十二指肠溃疡及预防复发 ,关键是根除

Objective:To study the effect of short term triple therapy on patients with Hp related duodenal ulcer Methods:Patients with Hp positive single duodenal ulcer,who were confirmed by gastroscopy and histology,were divided into one week group and maintenance group.Both groups received the triple therapy for one week.The one week group stopped having any treatment after the triple therapy.The maintenance group continued taking acid suppression therapy for 4 weeks after the triple therapy.All the patients had the gastroscopy and histology examination to check their ulcers and Hp eradication condition at 4 weeks and 1 year after the treatment finished Results:At one year after the triple therapy,all the patients received gastroscopy and Hp test.In the one week group,the ulcer healing rate was 83%,the Hp eradication rate was 75%.In the maintenance group,the ulcer healing rate was 94%,the Hp eradication rate was 80%.There was no obvious difference bertween the two gruops.At one year after the triple therapy,the ulcer healing rate in Hp eradicated group was 96%,while the ulcer healing rate in Hp uneradicated group was 64%.There was significant difference between the two groups Conclusion:The key point in treating duodenal ulcer and preventing its recurrence is eradication of Hp not maintenance of acid suppression therapy
