活性炭吸附阿霉素靶向治疗胃癌的临床观察

A Clinical Observation of Targeting Treatment with Adriamycin Activated Carbon Particle Delivery System in Human Gastric Carcinoma

目的 评价淋巴化疗制剂阿霉素 活性炭液 (ADM CH)的亲淋巴性和化疗效果。方法 制备ADM CH ;进展期胃癌 45例 ,分三组 ,每组 15例 ,每例注射含ADM10mg ;术前 1天经胃镜癌周注射ADM -CH和ADM者分别为靶向组和对照组Ⅰ ,外周静脉注射ADM者为对照组Ⅱ ;观察各组阿霉素浓度和临床及病理学疗效。结果 靶向组平均每例清除淋巴结数为 3 7 5枚 ,转移淋巴结病理学疗效为 45 1% ( 3 2 /71) ,第 1、2站淋巴结的阿霉素浓度分别为 3 2± 1 3和 2 7± 1 2 μg/g ,均较两对照组高 (P <0 0 5 ) ;靶向组术后半年生存率和淋巴结转移率分别为 93 3 %和 13 3 %。结论 阿霉素被活性炭吸附后癌周注射能靶向地进入淋巴结 ,较有针对性地治疗胃癌淋巴结转移 ,具有一定的临床应用价值。

Objective To estimate the effect of adriamycin activated carbon particle delivery system(ADM CH) on the treatment for human gastric carcinoma and its metastasis in lymph nodes. Methods 45 cases with advanced gastric carcinoma were divided into three groups randomly. Targeting group were injected ADM CH containing 10mg ADM into submucosa around tumor tissues under the direction of fiberscope ahead of 1 day for operation. Control groupⅠwere injected 10mg ADM with the same approach. And groupⅡ were injected 10mg ADM into vein. Results The average number of lymph node resected in patients was 37 5, and the effective percentage of pathology in lymph nodes metastasis was 45 1%(32/71). And the ADM concentration in D 1 and D 2 lymph node were 3 2±1 3 and 2 7±1 2μg/g respectively in the targeting group, which was significantly higher than control group(P<0 05). In targeting group, the survival rate for half year was 93 3%, and the metastasis rate in lymph node was 13 3%. Conclusions Adriamycin activated carbon particle delivery system is effective in the targeting treatment of human gastric carcinoma with lymph node metastasis.
