改良E WBH肝素化开始时间的探讨

摘要 目的探讨体外循环全身热灌注疗法ExtracorporealWholeBodyHyperthermia(EWBH)期间全身肝素化的开始时间对治疗的影响 ;方法采用前瞻性方法把 35例患者分为两组 ,其中Ⅰ组 15例患者使用先全身肝素化再双侧股静脉经皮穿刺插管。Ⅱ组 2 0例患者改为先双侧股静脉经皮穿刺插管再全身肝素化。然后在体外静脉—静脉转流下行EWBH ;结果对照两组的EWBH时间、肝素用量、鱼精蛋白用量、ACT值无明显变化 ;结论使用先股静脉插管成功后再全身肝素化的方法 ,对治疗过程没有影响 ,可有效预防穿刺部位出血并发症 ,使EWBH治疗更安全和可靠。 Objective To study the affect of heparinization starting time on curative effect During Extracorporeal Whole Body Hyperthermia (EWBH). Method 35 cases patients were divided into two groups by prospective way. 15 Patients in Group one were given double side femoral vein intubatton through skin puncture after whole body heparinization., 20 patients in group two were given double side femoral vein intubatton through skin puncture before whole body heparinization. Then conducted EWBH through in vitro vein to vein circulation. Results Compared the two group, there is no significant difference in EWBT time, heparin dosage, nucleoprotamine dosage and ACT value. Conclusion There is no curative effect by first given double side femoral vein intubatton through skin puncture then whole body heparinization, It will helpful in preventing bleeding in puncture location and ensure safety and reliability during EWBT treatment.
