妊娠肝内胆汁淤积症胆汁酸水平及羊水胎粪污染与胎儿宫内缺氧的关系 被引量:6

摘要 目的探讨妊娠肝内胆汁淤积症 (ICP)胆汁酸水平及羊水胎粪污染与胎儿宫内缺氧的关系。方法分别采用高效液相色谱法 (HPLC)及胆汁酸特异酶法比色测定 30例正常妊娠及 30例ICP胎儿脐动脉血的次黄嘌呤 (HX)及胆汁酸(TBA)水平。结果 (1)ICP缺氧组脐血TBA水平明显高于ICP无缺氧组 ,ICP无缺氧组高于正常妊娠组 [(2 3.770 0±11.8171) μmol/L ;(14 .85 5 0± 5 .4 5 95 ) μmol/L,(9.2 76 7± 4 .3888) μmol/L,P <0 .0 1]。 (2 )ICP组脐血HX水平与TBA水平呈正相关 (r=0 .6 89,P <0 .0 1)。 (3)ICP组中 ,有羊水胎粪污染者的脐血TBA水平及母血TBA水平 ,高于羊水清亮者。 (4 )ICP缺氧组的羊水胎粪污染率明显高于ICP无缺氧组 ,而ICP无缺氧组又高于正常组 [80 % (8/ 10 ) ,4 0 %(8/ 2 0 ) ,13.3% (4 / 30 ) ,P <0 .0 1]。结论ICP胎儿宫内缺氧可能是在绒毛间腔狭窄致胎盘氧储备力下降的基础上 ,母体循环、胎儿循环及羊水高浓度胆汁酸刺激胎盘绒毛血管及脐静脉收缩 ,致胎儿急性血氧灌注不足发生严重缺氧。
