影响血浆纤维蛋白原水平的多因素Logistic回归分析

摘要 目的 探讨影响血浆纤维蛋白原水平的环境与遗传因素 ;研究血浆纤维蛋白原在冠状动脉心脏病 ( CH D)发病中的作用。方法 选择 10 5例高纤维蛋白原者作病例 ,按 1∶ 1配比设置对照 ,探讨各种可能影响因素对血浆纤维蛋白原的影响 ;随访比较高纤维蛋白原组和低纤维蛋白原组人群心血管疾病的发生情况。结果 体重指数、高脂肪饮食、吸烟史、体力活动少、高血压、甘油三酯、高密度脂蛋白、低密度脂蛋白、一级亲属的血浆纤维蛋白原水平等 9个因素进入回归方程 ;高纤维蛋白原组一级亲属纤维蛋白原水平 ( 6.1± 0 .8) g/L 高于低纤维蛋白原组的一级亲属 ( 4.4± 0 .5 ) g/L;血浆纤维蛋白原浓度高者 CHD的发病率 ( 2 4% )高于血浆纤维蛋白原低者( 10 % )。结论 体重指数、高脂肪饮食、吸烟史、体力活动少、高血压、甘油三酯、低密度脂蛋白与血浆纤维蛋白原水平呈正相关 ,而高密度脂蛋白与血浆纤维蛋白原水平呈负相关 ;遗传对血浆纤维蛋白原水平有一定的影响 ;血浆纤维蛋白原浓度升高是 Objective To explore the effect of inheritary and environmental factors on plasma fibrinogen levels,identify plasma fibrinogen levels′ clinical significance in diagnosis of cardiovascular disease.Methods A case-control study of 105 subjects randomly selected from those who had higher plasma level of fibrinogen (group A),and 105 selected from those who had lower plasma level of fibrinogon (group B) was performed.Various factors influencing the plasma levels of fibrinogen between the groups A and B,and their incidences of cardiovascular disease were surveyed and followed up.Results Many environmental factors were found to affect fibrinogen levels,such as body weight index,high fat diet,smoking,little physical exercise,hypertension,triglycerides,and low-density lipoprotein;these factors were significantly positively correlated with plasma fibrinogen levels;but high-density lipoprotein was significantly negatively correlated with plasma fibrinogen levels.The above factors were processed by regression equation.The first-order relatives of group A had higher plasma fibrinogen level (6.1±0.8)g/L than those of group B (4.4±0.5)g/L;coronary heart disease (CHD) incidence was 24% in group A,while 10% in group B.Conclusion Environment and genetics all affect the plasma fibrinogen level;high plasma fibrinogen level is an independent risk factor for predicting CHD.
