6例Nucleus人工耳蜗植入临床分析

Analysis of the Nucleus cochlear implant in six cases

目的 :观察植入人工耳蜗装置后的疗效及言语康复效果。方法 :6例重度与极重度聋患者经面神经隐窝径路埋植Nucleus 2 2或Nucleus 2 4型人工耳蜗 ;术后给予数次调机及言语康复训练。结果 :植入手术全部成功。术后连续言语识别率由术前的 0 %～ 2 2 %达到 42 %～ 79.5 % ;言语清晰度从术前的 0 %～ 46%达到 5 3 %～ 81 %。3例患者术后完全可以利用听觉进行交流。结论 :Nucleus人工耳蜗植入术成功率高 ,术后经开机调试和言语康复训练 ,人工耳蜗装置工作状态正常。

Objective:To observe the speech&language skills rehabilitation of implanted multichannel cochlear. Methods: Six severe or profound sensory deaf patients were implanted with 22 or 24 channels cochlear (mode Nucleus 22 or 24) through facial nerve recess approach. Mapping of cochlear speech processor was done 1 month postoperatively, also after several times of speech processor adjustments and speech & language skills assessment were carried out. Results: Six cases were successfully implanted. Comparison of the continual speech recognition performance and speech clarity were improved from 0～22 % to 42～79.5 %, 0 ～46 % to 53 ～81 % , before and after the operation, respectively. Three cases could communicate satisfactorily when speaking slowly through telephone. Conclusion: Cochlear implantation in six Nucleus users showed that the operation was successful, the cochlear apparatus were well done after mapping speech processor and speech & language skills rehabilitation training.
