摘要
目的 :探讨原发性高血压(Ⅲ级)伴房性心律失常(AR ,含房性期前收缩或多源性房性期前收缩伴或不伴短阵房性心动过速)与P波离散度(Pd)及P波最大时限(Pmax)的关系。方法 :测定55例原发性高血压伴房性心律失常及55例非房性心律失常(对照组)的同步12导联心电图 ,P波最大时限(Pmax)和P波最小时限(Pmin) ,Pd=Pmax-Pmin。并进行比较。结果 :Pmax和Pd ,原发性高血压伴AR组为(122 6±8 9)ms和(50 4±8 2)ms;对照组为(106 2±7 8)ms和(31 8±6 9)ms两组比较有显著的统计学意义(p<0 01)。Pmax>110ms,Pd>40ms及两者联合预测AR的敏感性分别为83 6 % ,81 8%和80 0% ;特异性分别为87 3 % ,90 9 %和90 9 % ;阳性预示准确度分别为85 5 % ,86 4 %和85 5 %。结论
Objective:To study the relationship of essential hypertension accompany atrial arrhythmia P dispersion(pd) and maximum P wave duration(Pmax).Methods:The maximum P wave duration and the minimum P wave duration(Pmin) of 12-lead -electrocardiograms of 55 cases with essential hypertension accompany atrial arrhythmia and 55 cases non atrial arrhythmia,Analyse P wave dispersion.Results:The P max and Pd of essential hypertension accompany atrial arrhythmia group are 122 6±8 9ms and 50 4±8 2ms;It is 106 2±7 8ms and 31 8±6 9ms for control group.The difference is significant(p<0 01).Pmax 110ms,Pd 40ms and The sensitivities of both unite prediction essential hypertension accompany atrial arrhythmia are 83 6%,81 8%,and 80 0%;specificities are 87 3%,90 9%,and 90 9%;positive prediction accuracy are 85 5%,86 4%,and 85 5%.Conclusions:Pmax and Pd can prediction of essential hypertension accompany atrial arrhythmia reliable index.
