导致透析患者心脏病变原因的新认识 被引量:2

The current evaluation of cause of cardiac disease in dialysis patients

摘要 心血管疾病(CVD)是慢性维持性血液透析患者的主要并发症.当患者进入终末期肾肾病(ESRD)时,有相当多的患者出现心血管疾病的症状.美国肾脏病资料库显示,在3399例开始血液透析治疗的患者中40.8%已有缺血性心脏病.而在加拿大一项对822例开始血液透析患者多中心研究结果显示,心绞痛、心肌梗死、心力衰竭分别是21%、18%、35% .然而随透析治疗时间延长患病率呈升高趋势,在41个月的随访中,年新发生缺血性心脏病为3.6%,年新发生心力衰竭7.6%.ESRD住院患者中,约1/3的患者直接原因是心血管疾病,每年因心脏病及心肌梗死住院者为全部ESRD住院患者10%.心脏病也是ESRD患者死亡的首位原因,约占总死亡率的50%,美国维持性血液透析患者因心脏病死亡者为一般人群的35倍.
