产超广谱β-内酰胺酶菌在老年患者中的感染

ESBLs-producing Gram-negative Bacteria in Senile Patients: A Clinical Investigation

目的 了解产超广谱 β 内酰胺酶菌在老年患者中的感染情况 ,指导临床合理使用抗菌药物。方法 对1997年 1月～ 2 0 0 1年 10月 5 4 1份老年患者的各类标本中分离出的细菌进行分析 ,采用双纸片协同试验检测产超广谱 β 内酰胺酶菌株 ,纸片扩散法测定产超广谱 β 内酰胺酶菌株对 17种抗菌药物的敏感性。结果 产超广谱β 内酰胺酶菌株的总检出率 19.6 % ;产超广谱 β 内酰胺酶菌株对亚胺培南和头孢哌酮 /舒巴坦的敏感性达 10 0 %。结论 产超广谱β 内酰胺酶菌在老年患者中的感染率较高 ,临床宜选用亚胺培南和头孢哌酮 /舒巴坦治疗。

OBJECTIVE To study the infection condition of extended-spectrum β-lactamases(ESBLs) -producing Gram-negative bacilli in senile patients for reasonable use of antibiotics in clinic. METHODS The double-disk synergy test was performed to detect ESBLs-producing strains and 17 antibiotics were determined by the antimicrobial disk diffusion susceptibility tests in specimen collected from 741 senile patients. RESULTS The total rate of ESBLs-producing Gram-negative bacilli in senile patients was 28.7%. All strains of ESBLs were susceptible to imipenem and cefoperazone/ sulbactam(100%). CONCLUSIONS Imipenem and cefoperazone/ sulbactam were better for the infection caused by ESBLs-producing Gram-negative bacilli in senile patients.
