Kasai术式在高位胆管损伤修复中的应用(附五例报告)

Applying Kasai Way to Repair High Biliary Duct Injury (Report of 5 Cases)

目的 :探索修复高位胆管损伤的新术式。方法 :自 1995～ 2 0 0 1年 ,修复高位胆管损伤 5例 ,具体步骤 (1)游离损伤的近端胆管 ;(2 )将近端胆管套入空肠 ;(3)行肝门空肠Roux en Y吻合。结果 :5例均获得成功 ,皆无并发症 ,随访观察无 1例胆管狭窄。结论 :手术将损伤的胆管残端游离植进肠腔 ,可以有效地防止因胆肠直接吻合所致的吻合口狭窄 ;有助于避免胆道逆行感染 ;减少直接吻合所发生胆漏的机会 ;技术操作大为简便 ;不需要长期留置胆管支架。

Objective:To explore a new operative way for repairing biliary duct injury.Methods:(1) The injured proximal biliary duct was isolated;(2)The isolated proximal biliary duct were embedded in jejumum;(3)Roux en Y hepaticoportojejunostomy was performed.Results:All 5 cases were successfully treated jjjjjwihtout complication.Following up the patients,no biliary duct stricture was found.Conclusion:It can effectively prevent the biliary duct stricture,avoid retrograde infection,and reduce occurrence of biliary fistula.This operation is more simple and convenient.
