摘要 目的 观察霜天蛾过敏性哮喘患者特异性免疫治疗 (SIT)的疗效。方法 对 6 4例确诊为霜天蛾过敏性哮喘(PAA)患者注射霜天蛾变应原浸液 ,观察临床疗效和SIT前后皮内试验风团直径、淋巴细胞亚群 (CD4、CD8、CD4/CD8)、免疫球蛋白 (IgG、IgA、IgE)、肺功能 (FEV1、PEFR)及支气管激发试验 (PD2 0 FEV1)的变化。结果 SIT总有效率为 76 .5 % (49/ 6 4 )。SIT后皮内试验风团直径缩小 ,CD8细胞增加 ,CD4/CD8比例下降 ,IgE降低 ,IgG和IgA增高 ,FEV1、PEFR、PD2 0 FEV1均增加 ,(P值均 <0 .0 1)。结论 SIT对过敏性哮喘有较好的临床疗效 ,能增强患者的细胞免疫和体液免疫功能 ,改善肺通气功能 。 Objective To investigate the curative effect of specific immunotherapy (SIT) in patients with psilgramma menephore allergic asthma (PAA). Methods We investigated and contrasted the curative effect and laboratory indexes of 64 patients diagnosed definitely as PAA before and after the first year of SIT. The judgement standard of the curative effect was divided into clearly effective, effective, and noneffective. The laboratory indexes concerned lump's diameter by intradermal test, lymphocyte subsets (CD 4,CD 8,CD 4/CD 8) assay, immunoglobulin (IgG,IgA,IgE) assay, pulmonary function (FEV 1, PEFR) determination and bronchial provocative test (PD 20FEV 1) before and after SIT. Results The total effective rate of SIT was 76.5% (49/64). Lumps diameter by intradermal test, CD 4/CD 8, and IgE decreased. CD 8 lymphocyte, IgG and IgA, FEV 1 and PEFR, and PD 20FEV 1 increased ( P <0.01) after SIT. Conclusion SIT using psilgramma menephore allergen extract has a better curative effect in patients with PAA, and it can strengthen patients' humoral immunity and cell mediated immunity, improve pulmonary ventilatory function, and reduce bronchial hyperresponsiveness.
