血清基质金属蛋白酶9在急性胰腺炎严重性预测中的研究

Serious prognostic study of serum MMP-9 in acute pancreatitis

目的 探讨血清基质金属蛋白酶 9(MMP - 9)定量测定对急性胰腺炎严重性预测的价值。方法 对 1 0例健康志愿者 (CG)、1 0例急性充血水肿性胰腺炎 (MAP)、1 0例出血坏死性胰腺炎 (SAP)分别进行APACHEⅡ评分、血清中TNF -α、CRP、MMP - 9的浓度测定。结果 SAP组入院 1h血清MMP - 9的水平明显高于MAP和对照组 ,分别为P <0 0 1。SAP组入院时血清TNF、CRP的水平明显高于MAP和对照组 ,分别 P <0 0 1。病情稳定后其值明显下降 ,与入院时比较有显著差异 (P<0 0 1 )。同时分别行入院 1h的MMP - 9/TNF -α、MMP - 9/CRP、MMP - 9/APACHEⅡ相关分析具有显著意义 (相关系数0 95 6 ,P <0 0 1 ;0 935 ,P <0 0 1 ;0 95 7,P <0 0 1 )。结论 血清MMP - 9水平可能对AP具有一定的严重性预测价值 。

Objective To investigate the serious prognostic value of serum MMP-9 in acute pancreatitis.Methods Serum MMP-9 and CRP, TNF-α were determined quantitatively in 10 patients with SAP,10 patients with MAP,10 healthy persons,meantime made APACHE Ⅱ score,respectively.Results The serum MMP-9 level was significantly higher in SAP group than that in MAP group and control group in hospital at 1 hour,and was (470 8±51 4) ng/mL,(329 54±54 0) ng/mL,(246 5±26 8) ng/mL,respectively (P<0 01).The serum TNF,CRP levels were significantly higher in SAP group than those in MAP group and control group in hospital at 1 hour,(SAP/TNF-α 1 29±0 14 vs 0.87±0.13 MAP/TNF-α)μg/L, (SAP/TNF-α 1.29±0.14 vs 0.32±0.08 CG/TNF)μg/L and (MAP/TNF-α 0.87±0.13 vs 0 32±0 08 CG/TNF-α) ug/L and (SAP/CRP3 6±0 4 vs 0 9±0 1 MAP/CRP) μg/mL,(SAP/CRP 3 6±0 4 vs 0 7±0 10 CG/CRP) μg/mL and (MAP/CRP 0 9±0 1 vs 0 7±0 10 CG/CRP) μg/mL,respectively(P<0 01).When the state of illness were stable, they decreased.Compared with hospitalization, there was significance difference (P<0 01).Meanwhile correlation analysis had significantly meaning in MMP-9/TNF-α,MMP-9/CRP,MMP-9/APACHE Ⅱ in hospitalization (correlation coefficient 0 956,P<0 01),(0 935,P<0 01),(0 957,P<0 01).Conclusion Serum MMP-9 level might be valuable in serious prognosis and treatment in acute pancreatitis.
