改良双点牵张成骨术在下颌骨节段缺损修复中的应用

摘要 目的 探讨单一传送盘的两端牵张骨修复下颌骨节段缺损的可行性。方法 选用山羊6只,采用自行研制的下颌连续牵张器,经口外途径,将带牙传送盘两端进行顺序牵张成骨。结果 6只山羊的12~16mm长度下颌体的局段缺损被传送盘单端牵张修复,传送盘另一端再次牵张使下颌过度延长达到5mm左右,X线片示,缺损区和过度牵张区阻射情况良好,骨质连续性恢复;组织学显示牵张区有活跃新骨形成。固定6~8周,组织学和X线片可见新骨完全钙化,但改建仍未结束。结论 单一传送盘两端顺序牵张成骨修复下颌骨节段缺损是可行的。 Objectiv To explore the feasibility of bi-distraction osteogenesis of solo transport plate in mandibular segmental defect reconstruction. Method six goats were used in this study, extraoral distractor was activated 7 days after transport plate was completed between the distal trump and the plate with teeth at a rate of 1 mm/d for 12~16 days, then awaiting a cosolidation of 7days, activated it again between the transport plate and the medial trump. Result all the segmental mandibular defects were reconstructed at the distal of transport plates successfully with a extral lengthened 5 mm at the medial transport plates and the medial trumps. Xray and organ images confirm them. Conclusion it is feasible that us ing bi-distraction osteogenesis of a solo transport plate reconstructs the mandibular segmental defect.
