剖宫产率上升与新生儿窒息的关系

The ralations between the rising of cesarean section rate and the asphyxia neonatorum

目的 :探讨剖宫产率上升对新生儿窒息率、窒息程度及预后的影响。方法 :将 1994年～ 1999年剖宫产数按剖宫产率分成三个阶段 ,新生儿窒息率同步分组 ,对各阶段临床资料进行回顾性分析。结果 :剖宫产率逐阶段上升 (分别为 10 .5 3 %、2 5 .60 %、3 9.71% ) ,各阶段相比具有显著性差异 ( P<0 .0 5 ) ;新生儿窒息率逐步上升 ( 6.88%、7.61%、9.0 6% ) ( P>0 .0 5 ) ;剖宫产新生儿窒息率逐步上升 ( 4 .0 0 %、8.11%、12 .2 2 % ) ( P<0 .0 5 ) ;剖宫产窒息儿病死率逐渐上升 ( 0、1.67%、3 .3 3 % ) ( P<0 .0 5 ) ,新生儿窒息程度无明显改善 ( P>0 .0 5 )。结论 :合理掌握剖宫产指征 ,加强孕产期监护 ,提高产科医生技术水平 ,才能做到低剖宫产率、低新生儿窒息率。

Objective:To investigate the influence of the risin g of cesarean section rate with the incidence of asphyxia neonatorum and extent and prognosis.Methods:All the cesarean sections from 1994 to 1999 were divided into three phases according the incidence of cesarean section,the same with the incidence of asphyxia neonatorum.Clinical date were analyzed retrospectively.Results:The incidence of cesarean section was gone up by phase(10.53%?25.60%?39.71%),There was significantly difference(P<0.05).The incidence of asphyxia neonatorum was gone up(6.88%?7.61%?9.06%)(P>0.05).The incidence and mortality of asphyxia neonatorum in cesarean section was gone up(P<0.05).Th ere was signicantly difference.The extent of the asphyxia neonatorum was not in fluenced.Conclusions:It is advised to master the indication of cesarean section appropriately and to strengthen pregnant electronic monitoring and to increase technical competence of obstetrician can efficienty reduce the incidence of cesarean section and asphyxia neonatorum and mortality of neonatorum.
