摘要
探讨细胞因子 IL-1 ,IL-6,IL-8,TNF-α与胎儿宫内窘迫的关系及对分娩方式和产程长短的影响 .选择妊娠期正常、孕龄在 3 7周和 42周之间的单胎新生儿 1 2 8例 ,收集脐血样本 .将 1 2 8例按有无胎儿宫内窘迫及分娩方式分为 4组 ,采用免疫放射分析法测定 IL -1 ,IL-6,IL-8及 TNF-α.从产程的长短及分娩方式进行对比分析 ,并观察胎儿宫内窘迫脐血中细胞因子水平与 p H之间是否存在相关关系 .有无胎儿窘迫都不影响 IL-1脐血水平 .有胎儿宫内窘迫组的 IL-6和 IL-8水平显著高于无胎儿宫内窘迫组 (分别为 P=0 .0 1和 P<0 .0 0 1 ) ,而 TNF-α脐血水平显著降低 ( P<0 .0 0 1 ) .分娩可增加 IL-8,但对 IL-6水平没有影响 .若胎儿宫内窘迫与分娩因素并存 ,则能增加 IL-6和 IL-8水平 ,但 TNF-α水平降低 .细胞因子与 p H之间无相关关系 .胎儿宫内窘迫组 IL-6和 IL-8水平显著高于无胎儿宫内窘迫组 ,而 TNF-α脐血水平显著降低 .细胞因子与 p H之间无相关关系 .
The objective of the paper is to probe the relations between fetal distress and interleukin-1,interleukin-6 and interleukin-8,and to reveal the influence on the mode of delivery and the length of labour.Umbilical cord blood samples have been collected in a chosen group of 128 odd neonates of a pregnancy between 37 weeks and 42 weeks in a normal gestation.The group was divided into four subgroups according to the fetal distress and the mode of delivery.The serum levels of interleukin-1,interleukin-6,interleukin-8 and of tumor necrosis factor-α were determined by the means of irradiation immunoassays.The length of labour and the mode of delivery were correspondingly analyzed.The serum levels of interleukin and the pH in the cord blood of fetal distress neonates were also investigated to make sure whether there exists a correlativity among them.The results show that the presence or absence of fetal distress neither has influence on interleukin-1 levels.The IL-6 and IL-8 levels of the groups with the presence of fetal distress were significantly higher than that in the groups without fetal distress (P=0.01,P<0.001),while the cord blood TNF-α levels significantly decreased.Labour increased IL-8,but had no impact on IL-6.The IL-6 and IL-8 levels increased while TNF-α level decreased in the case of fetal distress and the labour.There was no correlation between interleukin and pH.So the IL-6 and IL-8 levels of the groups with fetal distress were significantly higher than that in the groups without fetal distress ,while the cord blood TNF-α levels significantly decreased.
