不同给药途径对大鼠急性胰腺炎治疗作用的差异

摘要 目的:探讨经不同途径给药对急性重症胰腺炎(SAP)大鼠不同脏器损伤的治疗效果。方法:SD大鼠46只,随机分为4组:①组1 对照组(假手术组)10只;②组2 急性出血坏死性胰腺炎(AHNP)经腹腔动脉给药组12只;③组3 AHNP经尾静脉给药组12只;④组4 AHNP组12只;用药组分别在造模后12h给药,持续8h,24h后取材,测血清磷脂酶A_2(PLA_2)浓度;观察肝、肺组织中肿瘤坏死因子-αmRNA(TNF-αmRNA)的表达变化,取部分肝、肺、胰腺组织作病理学检查。结果:与组4比较,组2门静脉血清PLA_2浓度明显降低(P<0.05),肝组织中TNF-αmRNA的表达减少(P<0.05),而肺组织中TNF-αmRNA的表达与组4差异无显著性(P>0.05);组3门静脉血清PLA_2浓度下降不明显(P>0.05),肺组织中TNF-αmRNA的表达减少(P<0.05),而肝组织中TNF-αmRNA的表达与组4无差异(P>0.05)。结论:药物对胰腺炎的治疗作用与给药途径相关,腹腔动脉灌注可减少SAP大鼠胰腺坏死面积,减低门静脉血PLA_2的活性,使肝组织中TNF-αmRNA的表达量明显下降,但对肺组织中TNF-αmRNA的表达影响小。外周静脉给药对肺脏有保护作用。 Objective: To observe the treatment effects on acute hemorrhagic necrotizing pancreatitis (AHNP) of rats through the different routes of administration from stern vein or from celiac trunk. Methods: Ten rats were set as group 1 (sham operation), another 36 healthy SD rats were randomly divided into 3 groups(12 rats for each group), among them, the rats of group 2 suffered AHNP with treatment administration from celiac trunk, Group 3 suffered AHNP with treatment administration from stern vein, group 4 suffered AHNP solely. At 12 hours after the induction of AHNP, the rats received intravenous or intra-artial drugs. The animals were sacrificed at 24 hours following AHNP. The portal blood samples were collected for measurement of serum phospholipase A2. The livers and lungs were partly taken for reverse transcription-polymerase chain reaction (RT-PCR) analysis of mRNA expression of tumor necrosis factor-alpha. The pancreases were partly taken for pathologic observation. Results: The portal serum phospholipase A2 and the TNF-αmRNA express in liver in group 2 were lower than those of group 3 and 4; but TNF-α mRNA expression in lung in group 3 was lower than that in group 2 and 4; pancreatic necrosis area in group 2 was smaller than that in group 3 and 4. Conclusion: The treatment effects on different organ injury depend on the route of adiministration. Drugs delivered through stern vein induced a greater reduction of lung TNF-α mRNA expression , drugs delivered through celiac trunk induced a greater reduction of liver TNF-aaaaaaaaaaa mRNA expression and were more effective in reviving pancreatic tissue.
