摘要
目的 :手术中设计一种使人工关节软骨与异体股骨髁关节面精确、紧密贴配的瞄准器。方法 :利用现代计算机和工程学知识 ,先设计并制造出内表面与异体股骨髁关节面轮廓一致的定位钻模实体 ;根据其 3个孔眼的大小、方向和位置关系 ,再设计出相应的辅助机械装置 ;并将它们用于大段同种异体骨关节移植的动物实验 8例。结果 :术后观察 16周 ,8只动物模型存活 6只 ,5只步态基本正常 ,1只不能站立行走。结论 :该瞄准器设计合理 ,定位准确 ,操作简便 ,解决了手术过程中假体与异体股骨髁部不易精确贴配的困难 ,为临床大段骨缺损的生物学重建提供了一种解决途径。
Objective: We designed a simple hand-held target device to facilitate the fit precisely and tightly between the artifical articular cartilage and the articular surface of allogenic distal femur.Methods:First,it was made a kind of localization model at the base of modern knowledge of computer and engineering,whose inter-surface was coincided with the articular surface of allogenic distal femur.Based on the relationship of the sizes,directions and positions of the model's three holes,counter-aided mechanical device was designed.Eight pigs were applied with this device in the test of allograft osteoarticular transplantation.Results:Only six pigs were alived sixteen weeks after the operation.Only one pig couldn't walk,and the others walked with basic normal gait.Conclusion:This device is designed for easy handling and precise orientation,which solved the difficult problem of the unfit precisely and tightly between the prosthese and femoral condyles.Thus,it provides a solution for biological reconstruction of clinically massive osteoarticular deficiency.
