背景:肠易激综合征(IBS)是肠道功能紊乱性疾病,其实验室诊断标准仅限于肠动力紊乱和一些介质的测定,缺乏针对肠道组织功能和病理生理变化的指标,尤其是肠黏膜上皮细胞功能变化的指标。目的:观察IBS模型大鼠肠黏膜上皮细胞功能的变化与临床表现的联系,建立肠道局部细胞功能变化的实验室诊断指标。方法:对肠黏膜上皮细胞进行分层刮片、荧光染色和还原能力测定,用原子力显微镜和激光共聚焦扫描显微镜测定细胞膜蛋白分子颗粒振荡和胞内pH值。结果:IBS模型大鼠的肠黏膜上皮细胞功能处于抑制状态,确定了与这种抑制状态相应的实验室诊断指标。对IBS患者结肠活检组织黏膜上皮细胞的检验初步证实了这一点。结论:初步建立了IBS肠黏膜上皮细胞功能变化的实验室诊断指标。
Background: Irritable bowel syndrome (IBS) is characterized by intestinal dysfunction, whose laboratory diagnostic indices are only restricted to disorders of intestinal motility and some hormonal analysis, but devoid of the indices of functional and pathophysiological changes of intestinal tissue, especially of the intestinal epithelial cells. Aims: To observe the relevance of functional changes of intestinal epithelial cells in IBS rat model and the clinical manifestations, and to set up the laboratory diagnostic indices of intestinal cellular functional changes. Methods: The intestinal epithelial cells in different layers were collected using slide techniques, fluorescence staining and reduction ability determination were performed. The atomic force microscope and laser scanning confocal microscope were used to explore the oscillation of cellular membrane protein molecules and intracellular pH value. Results: The functions of the intestinal epithelial cells in IBS rat model were in the inhibitory states. The laboratory diagnostic indices concerned with this inhibitory condition were found and confirmed by colonic mucosal biopsy specimens in IBS patients. Conclusions: The cellular functional indices of intestinal epithelium are suggested for laboratory diagnosis of IBS.
