巨大型垂体腺瘤手术入路的探讨——扩大前颅底经纵裂经平台入路的临床应用

垂体肿瘤 ; 显微外科手术 ; 治疗效果目的 通过手术入路和手术技术的改进以提高巨大垂体瘤的治疗效果。方法 采用扩大前颅底经纵裂经平台入路 ,对 10例侵入蝶窦且突入鞍上的巨大垂体瘤进行显微手术。结果 肿瘤全切除 8例 ,次全切除 2例 ,术后过程平稳。结论 采用此入路可提高巨大垂体瘤的手术效果 ,具有视野广阔、暴露良好。

Objective To introduce a new approach for improving the operative result of giant pituitary tumors. Methods Ten patients with giant pituitary tumors invading upward supra sellar and down into sphenoidal sinus were treated with the new approach: extensive frontal skull base translongitudinal fissure and sphenoidal platform sinus approach. Results The tumor was totally resected in 8 patients, subtotally resected in 2 patients,and the recovery process for all patients was smooth. Conclusion This operative approach can greatly improve the operative result. It has many advantages including good exposure, wide sight field within operative area,good protection of olfactory nerve and no serious complications.
