幽门螺杆菌与胸痛关系探讨

The relationship between Helicobacter pylori infection and chest pain

目的 探讨幽门螺杆菌 (H .pylori)感染时胸痛的发生与功能性消化不良 (FD)、冠心病(CHD)的关系。方法 采用胃镜检查 5 30例患者 ,钳取胃粘膜组织 ,用 Giem sa染色及 PCR基因扩增法检测 H .pylori,按 H.pylori阳性与否比较胸痛发生率 ,分析 FD、CHD与 H.pylori之间的关系。结果 H.pylori阴性中胸痛发生率为 13.0 % (39/ 30 0 ) ;H.pylori阳性中胸痛发生率为 19.6 % (4 5 / 2 30 ) ,两组比较差异有显著性 (P<0 .0 5 ) ;胸痛者 84人中 ,4 5例 H.pylori阳性中单 FD发生率 11.1% (5 / 4 5 ) ,单CHD发生率 33.3% (15 / 4 5 ) ,两者比较差异有显著性 (P<0 .0 5 )。在胸痛者 H .pylori阳性中 FD和 CHD均有的发生率为 31.1% (14 / 4 5 ) ,与 H.pylori阴性中 FD和 CHD均有的 5 .1% (2 / 39)相比差异有显著性 (P<0 .0 5 )。结论 H .pylori感染时 ,胸痛的发生可能与 FD和 CHD发病有关 ,而且二者还可能发生部分重叠。 H .pylori阳性患者出现胸痛时 ,一定要警惕 CHD可能 ;在治疗 H.pylori阳性 FD时 ,不能忽视 CHD的治疗。

Objective To assess the relationship between chest pain after Helicobacter pylori(H.pylori) infection and functional dyspepsia(FD) and coronary heart disease (CHD).Methods 530 cases were examined by the gastroscope and the biopsy mucosa were tested with GIEMSA stain and Polymerase chain reaction (PCR) for H.pylori infection.According to H.pylori positive and negative,the incidence of chest pain was analyzed.Results The incidence of chest pain in H.pylori negative patients was 13.0%(39/300),which was significantly different from 19.6%(45/230) in H.pylori positive patients (P<0.05).In the 84 cases with chest pain,the incidence of FD in 45 cases of H.pylori positive was 11.1%(5/45),which was significantly different from the incidence of CHD,33.3%(15/45),(P<0.05).The incidence of both FD and CHD in H.pylori positive patients with chest pain was 31.1%(14/45),which was significant different from the incidence of both FD and CHD in H.pylori negative patients,5.1% (2/39),(P<0.05).Conclusions THere may be close relationship between chest pain and FD and CHD in patients with H.pylori infection.H.pylori positive patients with chest pain must take CHD into consideration.
