胃癌全胃切除术后肠内营养对血浆游离脂肪酸的影响

摘要 目的探讨胃癌全胃切除术后肠内营养对血浆游离脂肪酸的影响。方法将30例胃癌行全胃切除术后的患者,随机分为两组:治疗(EN)组,术后第1天即开始行肠内高营养;对照(TPN)组,术后即给予全肠外营养,共9d。分别于术前及术后第1、5、9天,采静脉血,监测血浆中游离脂肪酸(C16:0、C18:0、C18:2、C20:4)。结果术前及术后d1两组无差异(P>0.05);d5、d9两组游离脂肪酸(FFA)均升高,C16:0、C20:4两组比较差异最明显(P<0.01);手术前后两组各自比较差异均有显著性(P<0.01)。结论手术创伤后血浆中游离脂肪酸浓度降低,术后EN能更快予以补充,并且优于TPN。 Objectives To explore the impact of enteral nutrition on plasma free fatty acids of the patients with total gastrectomy.Methods 30cases of patients with total gastrectomy were divided into two groups randomly.The treatment group patients were given early hyperalimentation from one day after operation.The control group patients were given parenteral nutrition immediately after operation to the ninth postoperative day.The venous blood was collected before operation and on the first,the fifth and the ninth postoperative day to monitor plasma free fatty acid s(C 16:0 ,C 18:0 ,C 18:2 ,C 20:4 ).Results There was no significant difference in the two groups before operation and on the first day after operation(P>0.05).The level of free fatty acids increased in the two groups on the third and the ninth days after operation when C 16:0 ,C 20:4 were significantly different between the two groups(P<0.01).The plasma C 16:0 ,C 20:4 in each group were also significantly different before and after operation(P<0.01).Conclusions The level of plasma free fatty acids decreased after operation because of trauma.The plasma free fatty acid could be brought up faster by EN than by TPN.
