电视内窥镜在喉显微外科手术中的应用

APPLICATION OF VIDEO ENDOSCOPE IN LARYNGOMICROSURGERY

目的 :评估电视内窥镜在喉显微外科手术中的应用价值。方法 :1 8例在传统显微喉镜下施行手术时由于暴露困难或存在观察盲区而无法完成手术的病例 ,改用电视内窥镜进行手术 ,总结手术效果。结果 :1 8例患者中 1 7例在电视内窥镜下顺利完成手术 ,效果满意。结论 :在支撑喉镜的基础上应用电视内窥镜进行喉显微外科手术 ,不强调支撑喉镜将会厌撑起到直视下完全暴露喉腔病灶 ,通过不同角度内窥镜就能全面观察喉腔情况 ,其视野图像清晰、彩色、动态 ,具有较强的立体感。电视内窥镜可以替代传统显微喉镜完成绝大多数显微手术 ,而且可以完成某些在传统显微喉镜因视野暴露不佳或存在观察盲区而无法完成的手术。

Objective:To evaluate the value of video endos cope applicated in laryngomicrosurgery . Methods:Eighteen cases, which had not been accomplished surgery with traditional microscopic techniques because of exposure difficulty or prese nce of blind areas in examination, were carried out operations with video endoscope an d the effects of surgery were summarized.Results:Seventeen cases were accomplished smoothly by this video endoscopic technique.Conclusion:When video endoscopic techniques were introduced into laryngomicrosurgery on the base of employing supporting laryngoscope, it is not necessary for the epiglottis to be supported to the strenth that the laryng eal cavity be exposed completely straight through the supporting laryngoscope; a nd the condition of laryngeal cavity can be observed fully with various visual a ngled endoscope. The image of video endoscope is of clear, color, dynamic and 3-D observation. In majority cases, video endoscope techniques can substit ute f or traditional microscopic techniques performing laryngomicrosurgery. Furthermor e, it can perform certain laryngomicrosurgery which can not be accomplished with traditional microscopic techniques.
