支气管肺泡灌洗治疗慢性阻塞性肺疾病的价值探讨

THE VALUE OF BRONCHOALVEOLAR LAVAGE TREATMENT FOR CHRONIC OBSTRUCTIVE PULMONARY DISEASES

目的 :探讨支气管肺泡灌洗治疗慢性阻塞性肺疾病 (COPD)患者的应用价值。方法 :A组使用氧氟沙星注射液及 0 .5 %甲硝唑注射液灌洗 ,局部滴入丁胺卡那 0 .2 ,地塞米松 5mg ,B组使用生理盐水灌洗 ,局部滴入丁胺卡那 0 .2。疗程 :每周 2次 ,连续 2周。观察两组患者治疗前后肺功能 ,血气分析 ,临床症状 ,生活质量等。结果 :A组治疗后FEV1 ,PFE高于治疗前及B组 ,B组虽有改善但不显著 (P >0 .0 5) ,AB两组治疗前后血气分析对比差异具显著性 (P <0 .0 5)。临床症状及生活质量改善A组明显于B组。结论 :支气管肺泡灌洗 +滴药可以改善COPD急性期肺功能及临床症状和生活质量 ,以使用抗生素灌洗加滴入糖皮质激素优于使用生理盐水灌洗 ,其操作简单易行、创伤少 ,只要严格把握适应证和禁忌证 。

Objective:To explore the value of bronchoalve olar lavage treatment for chronic obstructive pulmonary diseases (COPD). Methods:Group A using ofloxacin injection and 0.5% metronidazu linjection lavage, and then put drops of Amikacin 0.2 and dexamethason 5mg in t he focus of infection. Group B using physiological saline lavage and then put drops of Amikacin 0.2 in the focus of infection. The period of treatment was 2 weeks, twice a week. Lung function, blood-gas analysis, clinical symptom. Qu ality of life of two groups of patients were observation before and after treat ment. Results:FEV 1, PFE were higher after treatment than before tr eatme nt in group A, and there was no significant difference before and after treatmen t in group B ( P >0.05). There was significant difference of blood-gas analys is in both group A and group B before and after treatment ( P <0.05). Clini cal symptoms and quality of life were improved much higher in group A than tha t of group B.Conclusions:Fiberbronchoalscope lavage combined with put dro ps of medicine can improve acute period lung function. Clinical symptoms and qua lity of life of patients with COPD. Using anitibioties lavage combined with pu t drops of glucocorticoids was better than using physiological saline lavage, it was simple and easy to operate, and less trauma. To make sure the safty op eration. Strick application and contraindication should be emphysis.
