急性脑梗塞患者TNF-α与胰岛素抵抗关系的研究

目的 探讨急性脑梗塞患者胰岛素抵抗 (IR)和肿瘤坏死因子 (TNF -α)的改变及其相互关系。方法 检测急性脑梗塞 48例和正常对照组 3 6例的血糖 (FPG)、血清胰岛素 (FINS)、TNF -α的水平。结果 急性脑梗塞组的TNF -α、FINS、FPG比对照组高 (P均 <0 .0 1) ;血清TNF -α与FINS正相关 (r =0 .5 3 ,P <0 .0 1)。结论 急性脑梗塞病人可有IR和TNF -α水平增高 ,TNF -α与IR有密切相关性 ,两者可能共同参与缺血性脑血管病的发生和发展 ,拮抗TNF -α有可能改善胰岛素抵抗及减轻脑缺血患者脑损伤 ,对防治缺血性脑血管病具有一定的临床价值。

Objective To explore the changes and correlations of the insulin resistance (IR) and the tumor necrosis factor (TNF-α) in acute cerebral infarction patients. Methods 48 acute cerebral infarction patients and 36 in the control group were examined for the levels of the fasting plasma glucose (FPG), fasting insulin (FINS) and TNF-α. Results The levels of the TNF-α, FINS and FPG of the acute cerebral infarction patients were found to be higher than those of the control group (P<0.01); TNF-α and FINS were of positive correlation (r=0.53,P<0.01). Conclusion There may be level increases of IR and TNF-α in acute cerebral infarction patients, both are likely to play on the generation and development of cerebrovascular diseases. Antagonism TNF-α may help improve insulin resistance and reduce brain damages of cerebral ischemia patients, which prove to be of clinical value with regard to prevention and cure for ischemia cerebrovascular diseases.
