高龄食管癌和贲门癌的临床特点与外科治疗

The Clinical Characteristics and Surgical Treatment in Elderly Patients with Esophagus Cancer and Cardia Cancer

目的 探讨高龄食管癌和贲门癌患者的临床特点及外科治疗。方法 回顾分析 5 3例 70岁以上食管癌、贲门癌外科治疗的临床资料。结果 5 3例高龄患者中 41.5 %术前合并其他疾病 ,其中食管癌和贲门癌手术切除率分别为94.12 %、63 .15 % ,术后并发症发生率分别为 3 5 .2 9%、2 1.0 5 % ,术后病死率分别为 5 .88%、5 .2 6%。两者手术切除率比较差异有显著性 ,χ2 =6.2 3 66,P <0 .0 5 ,但并发症发生率及病死率比较差异无显著性 ,P 均 >0 .0 5。结论 高龄患者术前虽有较多并存病 ,但积极的围手术期处理 ,手术治疗仍是提高高龄食管癌。

Objective To explore the Clinical Characteristics and Surgical Treatment in elderly patients with esophagus cancer and cardia cancer. Methods The clinical data of 53 70-year-old and older patients with esophagus cancer and cardia cancer were retrospectively analyzed. Results 41.5% (53 patients) had other complicating diseases before surgery, among which, the excision rates of esophagus cancer and cardia cancer were 94.12% and 63.15% respectively and the death rate after operation were 5.88% and 5.26% respectively. There existed a significance in the comparison of the excision rates of the two groups with χ 2=6.2366, P<0.05 but without significance in the comparison of complication incidences and fatality rates, P>0.05. Conclusion Though there exist quite some complicating diseases in the elderly patients before surgery, operation still remains to be the most effective method to increase the excision rate and improve the quality of life in patients with esophagus cancer and cardia cancer if preoperation period is well prepared.
