摘要
目的 :探索跟骨定量超声 ( QUS)的临床应用价值。方法 :对骨科门诊自愿受试者进行 QUS测值及髋关节 X线摄片 ,由同一名放射专家行 Singh指数分级 ,随机取 48例 QUS主要参数及据 X线评估的 Singh指数 ,采用 SPSS软件作统计学处理 ,分析 QUS主要参数与 Singh指数的相关性 ,并建立判别方程 ,由 QUS的主要参数宽带超声衰减 ( BUA)、劲度指数 ( STI)及超声速率 ( SOS)求出 Singh指数。结果 :QUS主要参数中 ,BUA与Singh指数呈显著正相关 ( r =0 .85 7,P <0 .0 0 1 ) ,STI与 Singh指数呈显著正相关 ( r =0 .80 8,P <0 .0 0 1 ) ,而SOS与 Singh指数无显著相关性 ( r =-0 .2 2 9,P >0 .0 5 )。判别方程为 :F1 ( 1 ) =-2 40 7.3 3 5 +1 0 .999× BUA+3 .0 83× SOS-7.71 5× STI;F2 ( 2 ) =-2 3 99.92 6+1 0 .85 6× BUA+3 .0 78× SOS-7.60 5× STI;F3( 3 ) =-2 3 60 .5 1 2 +1 1 .1 1 7× BUA+3 .0 46× SOS-7.61 8× STI;F4 ( 4 ) =-2 3 3 7.466+1 1 .1 79× BUA+3 .0 2 5× SOS-7.5 1 3× STI;F5( 5 ) =-2 3 1 0 .60 3 +1 1 .2 2 7×BUA+3 .0 0 3× SOS-7.441×STI;F6 ( 6) =-2 3 1 7.5 0 0 +1 1 .2 2 9× BUA+2 .999× SOS-7.3 1 4× STI。结论 :QUS评价骨质量具有量化评估、省时、省钱、无辐射危害等优点 ,?
Objective: To explore the clinical value of calcaneus quantitative ultrasound (QUS). Methods: The volunteers of orthopaedic outpatient service were checked by QUS to obtain the main parameters, then checked by single hip joint x\|ray to obtain the Singh index which was consulted by a same specialist in radiology. 48 Cases were selected randomly and dealt with the soft ware of SPSS to analyse the correlation between the main parameters of QUS and Singh index. After establishing discriminant equations,we can calculate Singh index by broadband ultrasound attenuation(BUA), Stiffness index(STI) and speed of ultrasound(SOS). Results: Of the QUS main parameters, BUA has a significant correlation with Singh index (r=0 857, P<0 001), STI has a significant correlation with Singh index (r=0 808, P<0 001), while SOS has no significant correlation with Singh index(r=-0 229, P>0 05). Conclusion: Evaluating the quality and quantity of bone by QUS has advantages over Singh index. QUS having quantitative evaluation, sparing time and money and no radioactive damage, it can be used to evaluate the quality and quantity of bone comprehensively. QUS is a relatively scientific method, it can direct the treatment of fracture, especially in the choice of the internal implants. It can also direct the drug treatment for patients with osteoporosis.\;
