以二维和三维超声心动图诊断右室肥厚的对比研究

摘要 目的:对比研究二维和三维超声心动图技术诊断右室肥厚的敏感性和相对准确性。方法:在32例慢性肺源性心脏病患者中行经胸二维(2DE)和多平面经食管三维超声心动图(3DE)检查,分别测量其右室心肌重量,评价二种超声心动图技术对右室肥厚的阳性检出率,并以3DE所测右室心肌重量超过36g/m2(男性)和32g/m2(女性)为心肌肥厚标准,评价2DE对右室肥厚诊断准确率。结果:3DE对右室肥厚的阳性检出率为18.75%,2DE的检出率为15.63%。以3DE所测右室心肌重量为标准,2DE对左室肥厚的诊断准确率为84.38%(P<0.05),漏诊率为9.38%,误诊率为6.25%。结论:2DE在估测右室心肌重量方面准确性较差,应将3DE作为评价右室肥厚首选的诊断技术。 objective:To compare the sensitivity and the relative accuracy of two-dimensional(2DE)and three-dimensional(3DE)echocardiography in detecting right ventricular hypertrophy(RVH).Meth-ods:Thirty-two patients with chronic cor pulmonale were examined with transthoracic2DE and multiplane transesophageal3DE to measure the right ventricular mass.The diagnostic accuracy of2DE was evalueted a-gainst the right ventricular mass measured by multiplane transesophageal3DE.Results:With36g /m 2 (male)and32g /m 2 (female)as the standard value of right ventricular mass,the positive detecting rate of3DE was18.75%,and the parameter was15.63%in2DE.Using right ventricular mass determined by3DE as the standard value,the diagnostic accuracy of2DE was84.38%(P<0.05).Conclusions:Since the accuracy of transthoracic2DE is dissatisfactory,the3DE should be the first choice of diagnostic technique in assessing the patients with RVH.
