前后联合入路锁定钢板内固定治疗复杂性胫骨平台骨折的临床疗效

Effect of combined anterior and posterior approach locking plate internal fixation in treating complicated fracture of tibial plateau

目的探讨前后联合入路锁定钢板内固定治疗复杂性胫骨平台骨折的临床疗效。方法选取2012年1月—2014年7月合江县人民医院骨科收治的复杂性胫骨平台骨折患者65例,根据不同的手术入路方式分为联合入路组33例和前正中入路组32例。联合入路组患者予以前后联合入路锁定钢板内固定治疗,前正中入路组患者予以前正中入路锁定钢板内固定治疗。观察两组患者手术时间、术中出血量、术后引流量、Rasmussen膝关节功能评分(Rasmussen评分)及并发症发生情况。结果两组患者手术时间、术后引流量及Rasmussen评分比较,差异无统计学意义(P>0.05);联合入路组患者术中出血量多于前正中入路组,并发症发生率低于前正中入路组,差异有统计学意义(P<0.05)。结论前后联合入路锁定钢板内固定治疗复杂性胫骨平台骨折的临床疗效显著,且并发症少。

Objective To explore the effect of combined anterior and posterior approach locking plate internal fixation in treating complicated fracture of tibial plateau. Methods A total of 65 patients with complicated fracture of tibial plateau were selected in the People's Hospital of Hejiang County from January 2012 to July 2014,according to the different way of surgical ap-proach is divided into combined approaches group(33 case)and middle surgical approach group(32 case). Combined approa-ches group were given combined anterior and posterior approach locking plate internal fixation treatment,middle surgical ap-proach group were given middle surgical approach locking plate internal fixation treatment. Operation time,intraoperative blood loss,postoperative drainage flow,Rasmussen knee functional score(Rasmussen score)and occurrence of complications be-tween the two groups were compared. Results Operation time,postoperative drainage flow and Rasmussen score showed no significant differences between the two groups(P > 0. 05);intraoperative blood loss of combined approaches group was more than that of middle surgical approach group,complication rate of combined approaches group was lower than that of middle surgi-cal approach group(P < 0. 05). Conclusion Combined anterior and posterior approach locking plate internal fixation has nota-ble curative effect in the treatment of complicated fracture of tibial plateau,and has less complication.
