低分子肝素钙治疗儿童过敏性紫癜的疗效及其预防早期肾损害的作用

Effect of low molecular weight heparin calcium in treating henoch-schonlein purpura in children and role of prevention of early renal damage

目的探讨低分子肝素治疗过敏性紫癜(HSP)的临床疗效及其预防早期肾损害的作用。方法选择厦门市妇幼保健院儿科2011—2014年住院治疗的HSP患儿109例,应用随机数字表法分为对照组52例和观察组57例。对照组患儿予以综合治疗,观察组患儿在对照组基础上加用低分子肝素钙治疗。比较两组患儿临床疗效和尿微量清蛋白(m ALB)水平变化。结果观察组皮肤紫癜、腹痛消退时间短于对照组,皮疹反复发生率低于对照组,差异有统计学意义(P<0.05)。治疗前两组患儿m ALB水平比较,差异无统计学意义(P>0.05);治疗后观察组m ALB水平低于对照组,差异有统计学意义(P<0.05)。观察组紫癜性肾炎发生率低于对照组,差异有统计学意义(P<0.05)。结论低分子肝素钙治疗HSP,可有效缓解临床症状,有效预防早期肾损害的发生,且使用方便、不良反应少。

Objective To explore the effect of low molecular weight heparin calcium in treating henoch- schonlein purpura( HSP) in children and role of prevention of early renal damage. Methods A total of 109 children with HSP were selected in the Xiamen Maternal and Child Health Care Hospital from 2011 to 2014,according to the random grouping method,they were divided into observation group 57 cases and control group 52 cases. Control group were given combined therapy,observation group were given low molecular weight heparin calcium treatment on the basis of control group. Clinical effect and ry micro albumin( m ALB) level between the two groups were compared. Results Skin purpura,abdominal pain subsided time of observation group were shorter than those of control group,incidence of rash over and over again of observation group was lower than that of control group( P < 0. 05). Before treatment,m ALB level showed no significant differences between the two groups( P > 0. 05),after treatment,m ALB level of observation group was lower than that of control group( P < 0. 05). Incidence of henoch- schonlein purpura nephritis of observation group was lower than that of control group( P < 0. 05). Conclusion Low molecular weight heparin calcium has notable curative effect in the treatment of HSP,can effectively relieve the clinical symptoms,effectively prevent the happening of the renal damage,and easy to use,has less adverse reaction.
