微创股骨近端防旋髓内钉治疗老年股骨粗隆间骨折的临床疗效

Effect of minimally invasive proximal femoral nail anti-rotation in treating intertrochanteric fractures of the femur

目的探讨微创股骨近端防旋髓内钉(PFNA)治疗老年股骨粗隆间骨折的临床疗效。方法选取2013—2014年宜宾市第二人民医院收治的老年股骨粗隆间骨折患者78例,随机分为对照组与观察组,各39例。对照组患者行微创动力髋关节钉(DHS)治疗,观察组患者行微创PFNA治疗。观察两组患者手术时间、住院时间、骨折愈合时间、术中出血量、临床疗效及并发症发生情况。结果观察组患者手术时间、住院时间、骨折愈合时间短于对照组,术中出血量少于对照组,优良率高于对照组,并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论微创PFNA治疗老年股骨粗隆间骨折的临床疗效显著,且并发症少。

Objective To explore the effect of minimally invasive proximal femoral nail anti- rotation( PFNA) in treating intertrochanteric fractures of the femur. Methods A total of 78 patients with intertrochanteric fractures of the femur were selected in the Second People’s Hospital of Yibin from 2013 to 2014,they were randomly divided into control group and observation group,39 cases in each group. Control group were given minimally invasive dynamic hip screw( DHS) treatment,observation group were given PFNA treatment. Operation time,hospitalization time,healing time,intraoperative blood loss,clinical effect and occurrence of complications between the two groups were compared. Results Operation time,hospitalization time,healing time of observation group were shorter than those of control group,intraoperative blood loss of observation group was less than that of control group,excellent and good rate of observation group was higher than that of control group,complication rate of observation group was lower than that of control group( P < 0. 05). Conclusion Minimally invasive PFNA has notable curative effect in the treatment of intertrochanteric fractures of the femur,and has fewer complications.
