PCI伴冠心病家族史阳性患者1300例临床及冠脉病变特点分析

Clinical and coronary lesion characteristics in 1300 patients underwent PCI with family history of coronary heart disease

目的分析经皮冠状动脉介入治疗(PCI)伴冠心病(CHD)家族史阳性患者临床及冠脉病变特点。方法该院2006年12月-2014年6月13 800例行PCI患者中选取伴CHD家族史阳性者1300例,随机抽取无CHD家族史PCI患者1000例作为阴性组。对CHD家族史阳性组和阴性组临床及冠脉病变特点进行比较。结果在13 800例PCI患者中发现CHD家族史阳性者1300例,占PCI总数的9.4%。阳性组患者的BMI以及糖尿病、高血压病、左主干病变和单/双/三支病变发生率与阴性组无显著差异(P>0.05),发病年龄和吸烟患者发生率显著低于阴性组,而男性、高脂血症、AMI和开口病变、CTO发生率均显著大于阴性组,差异均有统计学意义(P<0.05)。结论 PCI患者中CHD家族史阳性率为9.4%,CHD家族史阳性患者中男性、高脂血症、AMI和复杂冠脉病变多于无家族史者。

Objective To explore the clinical and coronary lesion characteristics in patients with percutaneous coro-nary intervention(PCI)with family history(FH)of coronary heart disease(CHD). Methods A total of 13 800 patients,who underwent PCI,from December 2006 to June 2014 were enrolled in the study,screening patients with FH of CHD 1300 casea (positive group). Selected 1000 patients underwent PCI without FH of CHD as negative group. Compared the coronary artery disease and clinical characteristics of family history of CHD positive and negative groups. Results 1300 cases of positive fami-ly history of CHD were found among 13 800 cases of PCI patients,accounting for 9. 4% of the total PCI. Patients in positive group and negative group had no difference in BMI and the incidence of diabetes,hypertension,left main lesions and single/double/ three vessel lesions(P ﹥ 0. 05). The onset age and proportion of smokers of positive group were significantly lower than negative group(P ﹤ 0. 05). In positive group,the proportion of men and the incidence of hyperlipidemia,acute myocardial in-farction(AMI),ostial lesion,chronic total occlusion(CTO)were significantly greater than negative group,the difference were statistically significant(P ﹤ 0. 05). Conclusion The positive rate of FH of CHD in patients underwent PCI are 9. 4% . The proportion of men and the incidence of hyperlipidemia,AMI and complicated coronary artery lesions in patients with FH of CHD ere greater than those no FH of CHD.
