Effect of pulmonary surfactant combined with sildenafil and high- frequency oscillatory ventilation in the treatment of newborns with persistent pulmonary hypertension

摘要 目的探讨肺泡表面活性物质(PS)联合西地那非(SIL)和高频振荡通气(HFOV)治疗新生儿持续肺动脉高压(PPHN)的疗效。方法选取2013年6月—2015年6月在成都市妇女儿童中心医院新生儿科住院治疗并确诊PPHN患儿52例,根据治疗方法不同分为试验组(应用PS治疗)和对照组(未使用PS治疗),各26例。比较两组患儿的动脉血气分析指标〔动脉血氧分压(PO2),肺动脉压力(PAP)和吸氧分数(Fi O2)〕。结果治疗前两组患儿监测指标比较,差异无统计学意义(P>0.05);治疗后试验组PO2高于对照组,PCO2、Fi O2、PAP低于对照组,差异有统计学意义(P<0.05)。两组患儿均未发生严重不良反应。结论 PS联合SIL和HFOV治疗新生儿PPHN能快速缓解病情,有利于患儿预后。 Objective To explore the effect of pulmonary surfactant( PS) combined with sildenafil( SIL) and high- frequency oscillatory ventilation( HFOV) in the treatment of newborns with persistent pulmonary hypertension( PPHN).Methods According to the inclusion criteria,a total of 52 cases of PPHN infants were collected in Chengdu Womenand Children's Central Hospital from June 2013 to June 2015. They were divided into experimental group( use PS) and control group( without the use of PS) according to the different treatment,26 cases in each. The monitoring indexes( Blood gas,PAP and Fi O2) of the two groups were compared. Results There was no significant differences between the two groups before treatment( P > 0. 05). After treatment,monitoring indexe of PO2 in the experimental group was higher than the control group,but other monitoring indexes( PCO2,Fi O2 and PAP) were lower than the control group,the differences were statistically significant( P <0. 05). No serious adverse reactions occurred in the two groups. Conclusion PS combined with SIL and HFOV in the treatment of neonatal PPHN can quickly relieve the condition and conducive to the prognosis of children.
