彩色多普勒超声诊断胎儿脐带绕颈的临床价值

Clinical value of color doppler ultrasound in the diagnosis of fetal nuchal cord

目的探讨彩色多普勒超声诊断胎儿脐带绕颈的价值。方法选取2014年11月—2015年11月期郏县人民医院收治的产妇200例,均经彩色多普勒超声检查诊断为胎儿脐带绕颈,以最终分娩结果为判定依据。计算彩色多普勒超声检查诊断脐带绕颈的符合率,观察脐带绕颈与分娩方式及胎儿预后的关系。结果彩色多普勒超声检查诊断脐带绕颈的符合率为91.00%。胎儿脐带绕颈的羊水污染、Apgar评分<7、脐动脉S/D>3及剖宫产率高于脐带未绕颈,差异有统计学意义(P<0.05),胎儿脐带绕颈S值<0.085发生率与胎儿脐带未绕颈比较,差异无统计学意义(P>0.05)。结论彩色多普勒超声检查可对脐带绕颈、缠绕程度进行准确的诊断。

Objective To study the clinical value of color doppler ultrasound in the diagnosis of fetal nuchal cord.Methods From November 2014 to November 2015 in the People’s Hospital of Jia County 200 cases of maternal were selected,which were diagnosed nuchal cord by color doppler ultrasound. Birth outcomes was the criterion. Coincidence rate of nuchal cord diagnosed by color doppler ultrasound was calculated,relationship of nuchal cord and delivery way,fetus prognosis were observed. Results Coincidence rate of nuchal cord diagnosed by color doppler ultrasound 91. 00%. Amniotic fluid,Apgar score< 7,umbilical artery S/D > 3 cesarean section rate of fetus nuchal cord were higher than with the umbilical cord around the neck( P < 0. 05). S < 0. 085 incidence rate between fetus nuchal cord and umbilical cord around the neck showed no significant differences( P > 0. 05). Conclusion Color doppler ultrasound can accurately diagnose nuchal cord and wound degree.
