带锁髓内钉治疗四肢长骨干骨折的临床效果

Clinical effect of interlocking intramedullary nail in the treatment of long bone shaft fractures of limbs

目的探讨带锁髓内钉治疗四肢长骨干骨折的临床效果。方法选取保定市新市区医院2013年10月—2014年1月收治的四肢长骨干骨折患者90例,均为新鲜骨折患者。按照随机数字表法分为对照组与观察组,各45例。对照组患者采用钢板固定治疗,观察组采用带锁髓内钉治疗,比较两组患者的临床疗效、日常生活活动能力量表(ADL)评分及并发症发生率。结果 ADL评分时间和方法间无交互作用(P>0.05);时间间比较,差异有统计学意义(P<0.05);组间比较,差异无统计学意义(P>0.05)。两组患者总有效率比较,差异无统计学意义(P>0.05)。治疗前两组患者ADL评分比较,差异无统计学意义(P>0.05);治疗后8、12周观察组患者ADL评分低于对照组,差异有统计学意义(P<0.05)两组患者并发症发生率比较,差异无统计学意义(P>0.05)。结论带锁髓内钉治疗四肢长骨干骨折患者的临床效果确切,能够有效提高患者活动能力,是一种安全、有效的治疗方案。

Objective To explore the clinical effect of interlocking intramedullary nail in the treatment of the long bone shaft fractures of limbs. Methods A total of 90 patients with long bone shaft fractures of limbs were selected in the New Urban Hospital of Baoding from October 2013 to January 2014,all were fresh fractures. Patients were divided into control group and observation group by random number table,45 cases in each group. The control group were treated with plate fixation,the observation group were treated with interlocking intramedullary nail. The clinical efficacy,activities of daily living scale( ADL)score and the incidence of complications between the two groups were compared. Results There were not significant interaction time in ADL scores between treatment method and time( P > 0. 05); there were statistically significant differences in ADL scores among different time points( P < 0. 05); there were not statistically significant differences in ADL scores among groups( P > 0. 05). The total effective rate of two groups was compared,the difference was not statistically significant( P > 0. 05).Before treatment ADL scores between two groups showed no significant differences( P > 0. 05); 8,12 weeks after treatment the ADL scores in observation group were lower than the control group( P < 0. 05). Complication rate between the two groups showed no significant differences( P > 0. 05). Conclusion The clinical effect of interlocking intramedullary nail in the treatment of long bone shaft fracture of limbs is accurate,it can effectively improve the patient's ability of action,it is a safe and effective treatment.
