硝苯地平联合硫酸镁治疗妊娠高血压综合征的临床疗效

The clinical effect of nifedipine combined with magnesium sulfate in the treatment of pregnancy induced hypertension

目的探讨硝苯地平联合硫酸镁治疗妊娠高血压综合征的临床疗效。方法选取2013年1月—2016年1月玉山县仙岩镇中心卫生院收治的妊娠高血压综合征患者78例,随机分为研究组与对照组,各39例。对照组给予单纯硫酸镁治疗,研究组在对照组的基础上加用硝苯地平片口服治疗。治疗7d后,比较两组患者的临床疗效、血压改善情况[收缩压(SBP),舒张压(DBP)]、24h尿蛋白定量及母婴结局。结果研究组总有效率高于对照组,差异有统计学意义(P<0.05)。治疗前两组患者DBP、SBP及24h尿蛋白总量比较,差异无统计学意义(P>0.05);治疗后,研究组SBP、DBP及24h尿蛋白总量低于对照组,差异有统计学意义(P<0.05);治疗后两组患者SBP、DBP及24h尿蛋白总量低于治疗前,差异有统计学意义(P<0.05)。两组患者新生儿死亡率比较,差异无统计学意义(P>0.05);研究组宫缩乏力、产后出血、胎儿宫内窘迫及早产发生率均低于对照组,差异有统计学意义(P<0.05)。结论硝苯地平联合硫酸镁治疗妊娠高血压综合征的临床疗效确切,能有效解除血管痉挛,降低血压、控制蛋白尿,改善氧代谢功能及患者的临床症状。

Objective To investigate the clinical effect of nifedipine combined with Magnesium Sulfate in the treatment of pregnancy induced hypertension. Methods From January 2013 to January 2016 in Center Health Hospital of Xianyan Town 78 pregnancy induced hypertension were selected,which were randomly divided into study group and control group,39 cases in each group. The control group was treated with magnesium sulfate,study group was treated with nifedipine on the basis of the control group. 7d after treatment,the improvement of blood pressure [systolic blood pressure( SBP),diastolic blood pressure( DBP) ],24 h urinary protein and pregnancy outcome between the two groups were compared. Results The total effective rate of study group was higher than that of the control group,the difference was statistically significant( P < 0. 05). Before treatment DBP,SBP and 24 h content of urine protein between the two groups showed no significant differences( P > 0. 05); after treatment,SBP,DBP and 24 h urine total protein of the study group were lower than those of control group( P < 0. 05); after treatment SBP,DBP and 24 h urine total protein of the two groups were lower than before treatment( P < 0. 05). Neonatal death between the two groups was compared,there was no statistically significant difference( P > 0. 05); occurrence rate of uterine atony,postpartum hemorrhage,fetal distress and the probability of premature delivery in the study group were lower than the control group( P < 0. 05). Conclusion Nifedipine combined with magnesium sulfate have exact clinical effect in the treatment of pregnancy induced hypertension syndrome,can effectively relieve vascular spasm,reduce blood pressure,proteinuria,improve clinical symptoms and oxygen metabolism in patients.
