氢化可的松琥珀酸钠预防双腔管插管引起的支气管痉挛的效果

The effect of hydrocortisone sodium succinate in preventing bronchial spasm caused by double lumen tube intubation

目的探讨氢化可的松琥珀酸钠预防双腔管插管引起的支气管痉挛的效果。方法选取2014年3月—2016年3月江油市九○三医院收治的胸科左侧双腔管插管手术患者200例,根据治疗方法不同分为预输注氢化可的松琥珀酸钠组(Ⅰ组)和未用药组(Ⅱ组),各100例。Ⅰ组给予氢化可的松琥珀酸钠,Ⅱ组仅给予复方氯化钠注射液。比较两组患者单肺通气、双肺通气气道峰压、相哮鸣音发生率、SPO_2<96.0%发生率、痉挛发生率和痉挛程度。结果Ⅰ组单肺通气、双肺通气气道峰压低于Ⅱ组,差异有统计学意义(P<0.05)。Ⅰ组呼吸相哮鸣音发生率低于Ⅱ组,差异有统计学意义(P<0.05)。Ⅰ组患者SPO_2<96.0%发生率低于Ⅱ组,差异有统计学意义(P<0.05)。Ⅰ组患者支气管痉挛发生率低于Ⅱ组,差异有统计学意义(P<0.05);Ⅰ组患者轻度痉挛、中度痉挛、重度痉挛发生率低于Ⅱ组,差异有统计学意义(P<0.05)。结论静脉预输注氢化可的松琥珀酸钠可降低因双腔管插管引起的支气管痉挛发生率,减轻支气管痉挛程度。

Objective To study the effect of hydrocortisone sodium succinate in preventing bronchial spasm caused by double lumen tube intubation. Methods From March 2014 to March 2016 in the 903 Hospital of Jiangyou 200 patients with thoracic surgery left double lumen tube intubation were selected,according to the different treatment methods they were divided into hydrocortisone sodium succinate group（ Ⅰ group） and untreated group（ Ⅱ group）,100 patients in each group. Ⅰ group was treated with hydrocortisone sodium succinate,Ⅱ group was only used infusion of compound sodium chloride injection. One lung ventilation,two lung ventilation peak airway pressure,the incidence of wheeze,the incidence rate of SPO2 less than 96%,the incidence of spasm spasticity and degree of spasm spasticity were compared between the two groups. Results Single lung ventilation,lung ventilation airway peak pressure Ⅰ group of were lower than the Ⅱ group,the differences were statistically significant（ P < 0. 05）. Respiratory phase wheezing occurrence rate of I group was lower than the Ⅱ group,the difference was statistically significant（ P < 0. 05）. SPO2< 96% incidence rate of I group was lower than the Ⅱ group,the difference was statistically significant（ P < 0. 05）. Incidence of bronchial spasm in Ⅰ Group was lower than that of Ⅱ group,the difference was statistically significant（ P < 0. 05）; incidence of mild bronchial spasm,moderate bronchial spasm,severe bronchial spasm cramps in Ⅰ group were lower than Ⅱ group,the differences were statistically significant（ P < 0. 05）. Conclusion Intravenous infusion of hydrocortisone sodium succinate can reduce incidence of bronchial spasm caused by double lumen tube intubation,relieve degree of bronchial spasm.
