股骨近端锁定板与动力髋螺钉内固定治疗老年股骨粗隆间骨折的效果对比

Effect comparision of locking proximal femoral plate and dynamic hip screw on treating elderly femoral intertrochanteric fracture

目的比较股骨近端锁定板(LPFP)与动力髋螺钉内固定(DHS)治疗老年股骨粗隆间骨折(FIF)的临床效果。方法选取射阳县人民医院2013年2月—2015年6月收治的老年FIF患者80例,根据采用的术式不同分成DHS组36例与LPFP组44例。术后随访10个月,比较两组患者手术指标(手术时间、术中出血量、骨折愈合时间、住院时间)、并发症发生情况及临床效果。结果两组患者手术时间、术中出血量比较,差异无统计学意义(P>0.05);LPFP组骨折愈合时间、住院时间短于DHS组,差异有统计学意义(P<0.05)。LPFP组并发症发生率低于DHS组,差异有统计学意义(P<0.05)。LPFP组优良率高于DHS组,差异有统计学意义(P<0.05)。结论应用LPFP术式治疗老年FIF较DHS疗效好、恢复快、安全性高。

Objective To compare the effect of dynamic hip screw fixation( DHS) and locking proximal femoral plate( LPFP) in the treatment of elderly femoral intertrochanteric fracture( FIF). Methods A total of 80 FIF patients were selected in the People's Hospital of Sheyang County from February 2013 to June 2015,which were divided into DHS group( n =36) and LPFP group( n = 44) according to different surgical procedures. Two groups of patients were followed-up for 10 months after surgery,surgery indicators( including operative time,bleeding volum,fracture healing time,hospital stays),perioperative complications and clinical efficacy of two groups were compared. Results There was no statistically significant differences of operation time,bleeding volum between the two groups( P > 0. 05); hospital stays,fracture healing time of LPFP group were shorter than DHS group( P < 0. 05); incidence of complications of LPFP group was lower than DHS group( P< 0. 05); excellent rate of LPFP group was higher than DHS group( P < 0. 05). Conclusion Compared with DHS,LPFP surgical treatment in the treatment of elderly FIF has a better efficacy,with quick recovery and high safety.
