针灸辅助治疗神经根型颈椎病的临床疗效及其对患者椎动脉血流动力学的影响

Clinical effect of acupuncture assisted intervention for nerve-root type of cervical spondylosis and the impact on arteria vertebralis hemodynamics of patients

目的探讨针灸辅助治疗神经根型颈椎病的临床疗效及其对患者椎动脉血流动力学的影响。方法选取上海市奉贤区南桥镇光明社区卫生服务中心2016年12月—2017年11月收治的神经根型颈椎病患者106例,采用随机数字表法分为对照组和观察组,各53例。对照组患者采用基础疗法,观察组患者在对照组基础上联合针灸治疗,两组患者均持续治疗3个疗程。比较两组患者的临床疗效、治疗前后临床症状评分及左右侧椎动脉管径、血流速度、血流量。随访6个月,比较两组患者疾病复发情况。结果观察组患者临床疗效优于对照组(P<0.05)。治疗前,两组患者各项临床症状评分比较,差异无统计学意义(P>0.05);治疗后,观察组患者各项临床症状评分低于对照组(P<0.05)。治疗前,两组患者左、右侧椎动脉管径、血流速度和血流量比较,差异无统计学意义(P>0.05);治疗后,观察组患者左、右侧椎动脉管径、血流量大于对照组,血流速度快于对照组(P<0.05)。随访6个月,观察组患者疾病复发率低于对照组(P<0.05)。结论针灸辅助治疗神经根型颈椎病的临床疗效确切,其可明显缓解患者的临床症状及体征,改善椎动脉局部血供,降低远期复发率。

Objective To study the clinical effect of acupuncture assisted intervention for nerve-root type of cervical spondylosis and the impact on arteria vertebralis hemodynamics of patients.Methods A total of 106 cases of nerve-root type of cervical spondylosis were selected from December 2016 to November 2017 in Guangming Community Healthcare Service of Nanqiao Town,which were divided into control group and observation group according to random number table,53 cases in each group.The control group was treated with basic therapy,observation group was treated with acupuncture based on control group,both groups were treated for 3 courses.The clinical effect,clinical symptoms scores and diameter,blood flow velocity,blood flow volume of left and right side of vertebral artery before and after treatment were compared between the two groups.Followed-up for 6 months,the recurrence rate of two groups were compared.Results The clinical effect of observation group was higher than control group(P<0.05).Before treatment,no significant differences of clinical symptoms and signs scores was found between the two groups(P>0.05);after treatment,the clinical symptoms and signs scores of observation group were lower than control group(P<0.05).Before treatment,no significant differences of diameter,blood flow velocity,blood flow volume of left and right side of vertebral artery was found between the two groups(P>0.05);after treatment,the observation group of diameter and blood flow volume of left and right side of vertebral artery were higher than control group,blood flow velocity of left and right side of vertebral artery were faster than control group(P<0.05).Followed-up for 6 months,the recurrence rate of observation group was lower than control group(P<0.05).Conclusion Acupuncture assisted intervention have an exact clinical effect for nerve-root type of cervical spondylosis,which can obviously relief the clinical symptoms and signs of patients,improve local blood supply of vertebral artery,and reduce long-term recurrence rate.
