摘要 目的探讨不同剂量氨基酸营养策略在早产儿静脉营养中的应用效果。方法选取都江堰市人民医院2016年1月—2018年1月收治的早产儿80例,随机分为高剂量组和低剂量组,每组40例。患儿均进行新生儿监护管理及静脉营养,高剂量组患儿给予高剂量氨基酸营养策略,低剂量组患儿给予低剂量氨基酸营养策略。比较两组患儿生长发育情况(体质量恢复时间、体质量增加、最大体质量下降率、头围增长、住院时间与静脉营养时间)及前白蛋白、尿素氮、总胆汁酸、三酰甘油、胰岛素样生长因子1(IGF-1)水平。结果高剂量组患儿体质量恢复时间、住院时间及静脉营养时间短于低剂量组,体质量增加、头围增长大于低剂量组,最大体质量下降率低于低剂量组(P<0.05)。两组患儿总胆汁酸、三酰甘油水平比较,差异无统计学意义(P>0.05);高剂量组患儿前白蛋白、尿素氮、IGF-1水平高于低剂量组(P<0.05)。结论高剂量氨基酸营养策略在早产儿静脉营养中的应用效果优于低剂量治疗,其可更快速、有效地促进患儿生长发育,提高机体营养水平。 Objective To explore the application effect of different doses of amino acid nutrition strategy in preterm infants with parenteral nutrition.Methods A total of 80 cases of preterm infants were selected from January 2016 to January 2018 in Dujiangyan People’s Hospital,which were randomly divided into high-dose group and low-dose group,40 cases in each group.The high-dose group was given high doses of amino acid nutrition strategy,low-dose group was given lower doses of amino acid nutrition strategy.The growth and development(time of weight recovery,body mass gain,the maximum weight loss,head circumference growth,hospitalization time,intravenous nutrition time)and prealbumin,urea nitrogen,total bile acid,triglyceride,IGF-1 levels were compared between the two groups.Results The high-dose group of time of weight recovery,hospitalization time and intravenous nutrition time were shorter than low-dose group,body mass gain and head circumference growth were larger than low-dose group,the maximum weight loss was lower than low-dose group(P<0.05).No significant differences of total bile acid or triglyceride was found between the two groups(P>0.05);the high-dose group of prealbumin,urea nitrogen and IGF-1 levels were lower than low-dose group(P<0.05).Conclusion High doses of amino acid nutrition strategy has a better effect than low-dose in preterm infants with parenteral nutrition,it can more effectively promote the growth and development of children,and improve the nutritional level of the body.
