规则与非规则性肝叶切除术治疗肝胆管结石的效果比较

Comparison of the effects of regular and irregular hepatectomy for hepatolithiasis

目的比较规则与非规则性肝叶切除术治疗肝胆管结石的治疗效果。方法选择2010年8月-2016年8月医院收治的肝胆管结石患者80例,随机分为2组各40例。规则组行规则性肝叶切除术治疗,非规则组行非规则性肝叶切除术治疗,比较2组住院时长、手术出血量、术后输血率、术后并发症及总有效率。结果规则组住院时长短于非规则组,手术出血量少于非规则组,术后输血率低于非规则组,差异均有统计学意义(P<0.05)。规则组并发症总发生率为7.5%低于非规则组的35.0%,差异有统计学意义(χ~2=22.595,P<0.05)。规则组总有效率为97.5%高于非规则组的82.5%,差异有统计学意义(χ~2=12.500,P<0.05)。结论肝胆管结石患者应优先选择规则性肝叶切除术治疗,该手术疗法的安全性高于非规则性肝叶切除术,且对于患者术后康复更有利。

Objective To compare the therapeutic effects of regular and irregular hepatectomy for hepatolithiasis.Methods 80 patients with hepatolithiasis admitted to the hospital from August 2010 to August 2016 were selected,and they were randomly divided into two groups,40 cases in each group. The regular group underwent regular hepatectomy,and the irregular group underwent irregular hepatectomy,the length of hospital stay,the amount of surgical bleeding,postoperative blood transfusion rate,postoperative complications and total effective rate were compared.Results The length of hospitalization in the regular group was shorter than that in the irregular group,the amount of surgical bleeding was less than that in the irregular group,the postoperative blood transfusion rate was lower than that in the irregular group,and the differences were statistically significant(P<0.05).The overall incidence of complications in the regular group(7.5%)was lower than that in the irregular group(35.0%),and the difference was statistically significant(χ~2=22.595,P<0.05).The total effective rate of the regular group(97.5%)was higher than that of the irregular group(82.5%),and the difference was statistically significant(χ~2=12.500,P<0.05).Conclusion Patients with hepatolithiasis should be treated with regular hepatectomy, which is safer than irregular hepatectomy,and is more beneficial for postoperative rehabilitation.
