某三甲医院2016-2017年门急诊处方点评及不合理用药分析

Analysis of outpatient and emergency prescription in the hospital from 2016 to 2017

目的通过开展处方点评工作发现医院存在的不合理用药问题,为促进临床安全合理用药提供参考。方法随机抽查医院2016年1月-2017年12月门急诊处方,每月门急诊处方抽样率不少于总处方数的1‰,共抽查处方79 919张。结果 79 919张处方中不合理处方1 132张占1.42%。不合理处方中不规范处方占36.84%,不适宜处方占45.76%,超常处方占17.40%。处方点评不合理率由2016年全年平均3.76%下降至2017年全年平均0.54%。结论加强处方点评工作可规范临床医师用药行为,促进医院医疗质量提升,提高药师业务水平。

Objective To find out the problem of irrational drug use in the hospital through prescription review work,and to provide reference for promoting safe and rational drug use in clinic.Methods Randomly select the emergency department prescriptions from January 2016 to December 2017 in the hospital by random sampling and the sample rate of monthly emergency prescriptions is not less than 1‰,a total of 79 919 prescriptions were selected.Results Among the 79 919 prescriptions,1 132 prescriptions were unreasonable,accounting for 1.42%of the total.Irregular prescriptions account for 36.84%of unreasonable prescriptions,inappropriate prescription accounted for 45.76%,supernormal prescriptions accounted for 17.40%.The unreasonable rate of prescription reviews dropped from an average of 3.76%in 2016 to an average of 0.54%in 2017.Conclusion Strengthening the prescription review can ensure the safety of patients,standardize the behavior of clinicians,promoting the quality of hospital medical care,and improve the professional level of pharmacists.
