氨氯地平联合厄贝沙坦治疗社区老年冠心病合并高血压患者的效果观察

Effect of amlodipine combined with irbesartan on elderly patients with coronary heart disease and hypertension in community

目的观察氨氯地平联合厄贝沙坦治疗社区老年冠心病合并高血压患者的临床效果。方法将社区全科治疗的老年冠心病合并高血压患者108例随机分为观察组和对照组,每组54例。对照组单用氨氯地平治疗,观察组采用氨氯地平联合厄贝沙坦治疗,比较2组临床疗效、血压及血脂水平变化、不良反应情况。结果观察组治疗总有效率为96. 30%,高于对照组的74. 07%(P <0. 01);治疗后,2组收缩压(SBP)、舒张压(DBP)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL-C)数值较治疗前均下降,高密度脂蛋白(HDL-C)较治疗前升高,且观察组降低或升高幅度大于对照组(P均<0. 01);2组心悸、头晕、恶心、下肢水肿等不良反应发生率比较差异无统计学意义(P>0. 05)。结论氨氯地平联合厄贝沙坦治疗社区老年冠心病合并高血压患者的效果显著,降压降脂效果良好,能有效抑制病情进展,且安全性较高,具有积极的临床意义。

Objective To observe the effect of amlodipine combined with irbesartan on elderly patients with coronary heart disease and hypertension in community.Methods 108 elderly patients with coronary heart disease and hypertension were randomly divided into observation group and control group,54 cases in each group.The control group was treated with amlodipine alone,while the observation group was treated with amlodipine combined with irbesartan.The clinical efficacy,blood pressure,lipid level and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 96.30%,which was higher than 74.07%of the control group(P<0.01).After treatment,the levels of SBP,DBP,TC,TG and LDL-C in the two groups decreased and HDL-C increased compared with those before treatment,and the decrease or increase in the observation group was greater than that in the control group(all P<0.01).There was no significant difference in the incidence of adverse reactions such as palpitation,dizziness,nausea and lower limb edema between the two groups(P>0.05).Conclusion Amlodipine combined with irbesartan in the treatment of elderly patients with coronary heart disease and hypertension in the community has a significant effect.The effect of lowering blood pressure and lipid is good.It can effectively inhibit the progress of the disease,and has a high safety.It has a positive clinical significance.
