腹腔镜胆囊切除术与开腹胆囊切除术治疗胆结石效果的比较研究

Comparative study on the clinical effects of laparoscopic cholecystectomy and open cholecystectomy for gallstones

目的比较腹腔镜胆囊切除术与开腹胆囊切除术治疗胆结石的临床效果。方法选取德阳第五医院2017年1月—2018年6月收治的胆结石患者140例,按照随机数字表法分为开腹组与腹腔镜组,各70例。开腹组患者予以开腹胆囊切除术治疗,腹腔镜组患者予以腹腔镜胆囊切除术治疗。比较两组患者临床效果、手术治疗指标(手术时间、出血量、肠道功能恢复时间、住院时间及镇痛药物使用率),并观察两组患者并发症发生情况。结果腹腔镜组患者治疗总有效率高于开腹组(P<0.05)。两组患者手术时间比较,差异无统计学意义(P>0.05);腹腔镜组患者出血量少于开腹组,肠道功能恢复时间、住院时间短于开腹组,镇痛药物使用率低于开腹组(P<0.05)。腹腔镜组患者并发症发生率低于开腹组(P<0.05)。结论与开腹胆囊切除术比较,腹腔镜胆囊切除术治疗胆结石的临床效果更好,可提高结石清除率,缩短肠道功能恢复时间,且安全性较高。

Objective To compare the clinical effects of laparoscopic cholecystectomy and open cholecystectomy for gallstones.Methods A total of 140 cases of patients with gallstones were selected from January 2017 to June 2018 in Deyang Fifth Hospital,which were randomly divided into open-abdominal group and laparoscopy group,70 cases in each group.The open-abdominal group was treated with open cholecystectomy,and the laparoscopic group was treated with laparoscopic cholecystectomy.Clinical effects,surgical treatment index(operation time,bleeding volume,intestinal function recovery time,stay time and analgesic drug use rate),and the incidence of complications were observed.Results The total effective rate of laparoscopy group was higher than open-abdominal group(P<0.05).There was no significant difference in the operative time of the two groups(P>0.05).The laparoscopy group of bleeding volume was less than open-abdominal group,intestinal function recovery time,stay time were shorter than open-abdominal group,analgesic drug use rate was lower than openabdominal group(P<0.05).The incidence rate of complications in laparoscopy group was lower than open-abdominal group(P<0.05).Conclusion Compared with open cholecystectomy,the clinical effect of laparoscopic cholecystectomy is better for the gallstones,it can improve stone clearance and shorten intestinal function recovery time,and with high safety.
