金华市中心医院静脉用药调配中心常见不合理用药情况分析

Analysis of common unreasonable drug use in pharmacy intravenous admixture services of Jinhua Municipal Central Hospital

目的分析金华市中心医院静脉用药调配中心(PIVAS)常见不合理用药情况。方法收集金华市中心医院2016年7—12月由专职审方药师审核出的不合理用药医嘱171例,分析不合理用药医嘱类型及其内容。结果不合理用药医嘱类型有配伍不适宜,其次为单次或单日给药剂量错误、医嘱录入错误、溶媒选择错误、溶媒剂量错误、用法错误。结论本院不合理用药医嘱类型较多,因此,药师应有效地拦截不合理医嘱,科学合理安排给药批次,并多次审核医嘱的合理性,将其及时反馈给临床,可促进临床合理用药。

Objective To analyse the common unreasonable drug use in pharmacy intravenous admixture services of Jinhua Municipal Central Hospital.Methods A total of 171 unreasonable medication orders were collected from the fulltime prescription pharmacists from July to December 2016 in Jinhua Municipal Central Hospital,and the types and specific contents of unreasonable medication orders were analyzed.Results The types of irrational medication doctor’s advice are incompatibility,followed by single or single-day dosage errors,doctor’s advice input errors,solvent selection errors,solvent dosage errors,usage errors.Conclusion There are many types of irrational medication orders in our hospital.Therefore,pharmacists should effectively intercept irrational medication orders,arrange medication batches scientifically and rationally,and check the rationality of medical orders many times,and give them back to the clinic in time,so as to promote rational medication in clinic.
