闭合复位微创锁定钢板内固定治疗胫腓骨多段骨折临床观察

Clinical observation of closed reduction and minimally invasive locking plate internal fixation in treatment of humeral multi-segment fractures

目的观察闭合复位微创锁定钢板内固定治疗胫腓骨多段骨折的治疗效果。方法选取2015年9月-2016年8月在医院进行胫腓骨多段骨折治疗的患者208例,随机分为研究组及常规组各104例,对常规组患者行传统切开复位钢板内固定法,对研究组患者行闭合复位微创锁定钢板内固定法。比较2组患者治疗情况、愈合时间、治疗效果,术后1年时生活质量及并发症情况。结果研究组患者的手术时间、术中出血量及住院时间均显著优于常规组,治疗优良率为82.69%显著高于常规组的62.50%,骨折愈合时间为(179.26±9.67)d短于常规组的(415.61±19.98)d,差异有统计学意义(P<0.05)。术后1年,研究组患者在总体健康、生理功能、生理职能及躯体疼痛等8项生活质量评分均显著优于常规组,研究组术后并发症发生率为2.88%明显低于常规组的45.19%,差异有统计学意义(P<0.05)。结论对胫腓骨多段骨折患者采用闭合复位微创锁定钢板内固定法进行治疗,能有效提高治疗效果,促进其骨折部位的愈合,并能降低患者因治疗而受到的创伤,降低其治疗成本,且对促进其生活质量的提高具有重要意义,值得推广。

Objective To observe the therapeutic effect of closed reduction and minimally invasive locking plate internal fixation in treatment of humeral multi-segment fractures.Methods 208 patients with humeral multi-segment fractures were treated in the hospital from September 2015 to August 2016,they were randomly divided into study group and conventional group,each of 104 cases.Conventional group of patients underwent conventional open reduction and internal fixation with plate fixation,and the patients in study group underwent closed reduction and minimally invasive locking plate internal fixation.The treatment status,healing time,treatment effect,and quality of life and complications at 1 year after surgery were compared between two groups.Results The operation time,intraoperative blood loss and hospitalization time of study group were significantly better than conventional group,the excellent and good rate of treatment was 82.69%,which was significantly higher than 62.50%of conventional group,and the fracture healing time was(179.26±9.67)d shorter than(415.61±19.98)d of conventional group,the difference was statistically significant(P<0.05).One year after operation,the eight quality of life scores of study group were significantly better than conventional group in terms of overall health,physiological function,physiological function and physical pain;The postoperative complication rate of study group was 2.88%,which was significantly lower than 45.19%of conventional group,the difference was statistically significant(P<0.05).Conclusion The treatment of multi-segment fractures of the humerus with closed reduction and minimally invasive locking plate internal fixation can effectively improve the treatment effect,promote the healing of the fracture site,and reduce the trauma of the patient due to treatment,reducing the cost of treatment.It is of great significance to promote the improvement of its quality of life and is worth promoting.
