摘要
结合文献回顾性分析临床药师参与3例患者治疗过程,临床药师结合患者血药浓度检测结果及临床资料和用药信息,及时调整患者给药方案,以构建标准化的治疗药物检测服务平台,为患者进行个体化的药学监护,从而保障患者用药安全有效。
Based on the literature review,clinical pharmacists participated in the treatment of 3 patients.Clinical pharmacists adjusted the drug delivery plan in time according to the results of drug concentration test and clinical data and drug information,so as to build a standardized service platform for drug detection and provide individual pharmaceutical care for patients.In order to ensure the safety and effectiveness of drug use in patients.
