应用三维电解剖标测系统零X线透视下射频消融治疗阵发性室上性心动过速 被引量:6

Non-X-ray fluoroscopic radiofrequency catheter ablation for treating paroxysmal supraventricular tachycardia guided by 3D electroanatomical mapping system

摘要 目的对比Ensite Velocity三维电解剖标测系统零X线曝光与常规X线曝光下对阵发性室上性心动过速(PSVT)行导管射频消融治疗(RFCA)的可行性,探讨三维电解剖标测系统在PSVT诊疗中的应用价值。方法入选拟行RFCA的PSVT患者86例,随机分为2组:三维电解剖组(45例)应用电解剖标测系统零X线透视下标测治疗,常规透视组(41例)应用X线透视下标测治疗。比较两组患者手术时间、X线透视时间、即时成功率、并发症发生率、随访成功率的差异。结果与常规透视组相比,三维电解剖组手术时间类似(P>0.05),X线透视时间明显减少(P<0.05),手术即时成功率及随访成功率明显提高(P<0.05),并发症发生率明显降低(P<0.05)。结论三维电解剖标测系统在阵发性室上性心动过速射频消融治疗中安全、高效,可尝试应用以最大限度减少X线透视。 Objective To compare the feasibility of non-X-ray fluoroscopic radiofrequency catheter ablation(RFCA) and routine X-ray fluoroscopic RFCA for treating paroxysmal supraventricular tachycardia(PSVT) guided by Ensite Velocity 3D electroanatomical mapping system, and discuss the application value of 3D electroanatomical mapping system in diagnosis and treatment of PSVT. Methods The patients with PSVT and planed RFCA(n=86) were chosen and randomly divided into 3D group(n=45) given treatment of non-X-ray fluoroscopic RFCA guided by Ensite Velocity 3D electroanatomical mapping system, and routine group(n=41) given treatment of X-ray fluoroscopic RFCA. The procedure time, X-ray fluoroscopic time, immediate success rate, incidence of complications and follow-up success rate were compared between 2 groups. Results The procedure time was similar(P>0.05), X-ray fluoroscopic time decreased significantly(P<0.05), immediate success rate and follow-up success rate increased significantly(P<0.05) and incidence of complications decreased significantly(P<0.05) in 3D group compared with routine group. Conclusion 3D electroanatomical mapping system is safe and effective in treatment of PSVT with RFCA, which can be tried to apply for minimizing X-ray fluoroscopy.
